Pediatric Plasma and Platelet Transfusions on Extracorporeal Membrane Oxygenation: A Subgroup Analysis of Two Large International Point-Prevalence Studies and the Role of Local Guidelines*

Conclusions: Children supported by extracorporeal membrane oxygenation receive large volumes of plasma and platelet transfusions with some institutional guidance in the form of protocols, but significant variation in practice. Interventional studies are necessary to provide evidence to direct the transfusion of hemostatic products in children supported by extracorporeal membrane oxygenation.

Authors: Mujer MTP, Rai MP, Atti V, Dimaandal IL, Chan AS, Shrotriya S, Gundabolu K, Dhakal P Abstract Non-vitamin K antagonist oral anticoagulants (NOACs) include thrombin inhibitor dabigatran and coagulation factor Xa inhibitors rivaroxaban, apixaban, edoxaban, and betrixaban. NOACs have several benefits over warfarin, including faster time to the achieve effect, rapid onset of action, fewer documented food and drug interactions, lack of need for routine INR monitoring, and improved patient satisfaction. Local hemostatic measures, supportive care, and withholding the next NOAC dose are usually sufficient to achie...

Authors: Bazarbashi N, Miller M Abstract INTRODUCTION: Icosapent Ethyl (IPE) is a highly purified (>96%) form of eicosapentanoic acid, a marine-derived omega-3 fatty acid known to reduce serum triglyceride levels. Experimental and human studies also support anti-atherosclerotic properties of IPE including antioxidant, anti-inflammatory, endothelial and plaque stabilizing effects. In the Reduction of Cardiovascular Events with Icosapent Ethyl-Intervention Trial (REDUCE-IT)l),the addition of 4 grams IPE daily resulted in a 25% reduction in cardiovascular events beyond statins and other standard of care therapies. ...

Most randomized clinical trials (RCTs) assessing the effects of initial anti-infective periodontal therapies use surrogate outcomes as primary outcome variables, such as mean changes in probing depth (PD) or in clinical attachment. However, these parameters do not reflect...

CONCLUSIONS: Non-surgical periodontal therapy was effective during the first 18 months, but periodontal conditions were worse at 48 months after therapy. The distal sites of abutment teeth with direct retainers presented the worst periodontal conditions. PMID: 32224546 [PubMed - in process]

ConclusionsConversion of prior Nissen fundoplication to RnY anatomy is technically challenging, although it is safe and feasible even in the setting of multiple prior foregut operations. A minimally invasive approach should be offered to patients by surgeons with experience in revisional foregut and bariatric surgery.

ConclusionsIntracorporeal anastomosis using linear staplers reduced anastomotic bleeding and stenosis compared to extracorporeal anastomosis after LTG. Future research will be required to determine the ideal method for intracorporeal anastomosis in LTG.

Nov ák, Tibor / Németh, Gábor / Kozinszky, Zoltan / Urbán, Edit / Gorzó, István / Radnai, MártaPage 165 - 170Purpose: The aim of the present study was to analyse the role of the main diagnostic signs of poor periodontal status, probing depth (PD) ≥4 mm and bleeding on probing (BOP) ≥50%, both simultaneously and individually in preterm birth (PB) and their effect on the birth weight (BW). Prematurity is a major health concern and it is the leading contributing factor to neonatal morbidity and mortality worldwide. Conflicting results exist on the relation between poor ma...

Acquired hemophilia A (AHA) is a rare autoimmune disease due to acquired IgG1-4 autoantibodies against coagulation factor VIII (FVIII). This causes patients, who have negative family and personal histories, to present with a sudden onset of heavy bleeding.1 –3 Although rare, the presence of antibodies against factors V and IX is possible.4

Acquired platelet dysfunction with eosinophilia (APDE) is a transient bleeding disorder characterized by spontaneous bruising in a healthy person. Spontaneous ecchymosis appears in 100% of patients with APDE.1 Other symptoms include epistaxis, gum or oral bleeding, gastrointestinal bleeding, hematuria, bleeding of the genital organs, subconjunctival hemorrhage, and excessive bleeding after tooth extraction. The diagnosis of APDE requires three elements: (1) clinical manifestation with spontaneous ecchymosis on the trunk or extremities, (2) hemogram showing eosinophilia, and (3) platelet dysfunction.
