Viral triggers have been implicated in the pathogenesis of cardiac allograft vasculopathy (CAV), the Achilles heel of heart transplantation (HT). Cytomegalovirus is associated with impaired coronary endothelial function, and in pediatric HT patients a variety of viruses, particularly adenovirus, are associated with graft dysfunction or loss. Hepatitis B virus (HBV) is associated with vasculitis and vascular processes in non-HT patients. We thus investigated the association between HBV seropositivity and CAV after HT.

Cardiac gene therapy remains limited by inefficient transgene delivery with currently available methods (intravenous infusion, transcatheter coronary artery infusion, direct myocardial injection). Viral infection remains a major cause of morbidity and mortality after allo-HSCT (Bollard/Heslop Blood 2016). Antibiotics for treatment of viral infection in immunocompromised patients are limited in efficacy and associated with toxicity (Gerdemann BBMT 2004; Sili Cytother 2012). Increasing evidence suggests that use of VST for immunocompromised patients with viral infections can provide therapeutic benefit and improve OS (Bollard/Heslop Blood 2016; Sutrave Cytother 2017). Methods of VST production include ex-vivo expansion and direct selection (Gottlieb Cytother 2017).
