Factors driving olfactory loss in patients with chronic rhinosinusitis: a case control study

BackgroundOlfactory dysfunction (OD) in chronic rhinosinusitis (CRS) is common. It is likely that numerous factors such as sex, race, age, allergies, asthma, smoking, and other comorbidities play a role in CRS ‐related OD. In order to determine which aspects of OD are due solely to CRS and which are associated with other confounders, control populations are needed to allow appropriate risk assessments.MethodsProspective, multi ‐institutional enrollment of patients with CRS and control subjects without CRS was performed. Demographic information, comorbidities, and olfactory testing (Sniffin’ Sticks) of threshold (T), discrimination (D), and identification (I) scores (TDI) was collected.ResultsA total of 224 patients with CRS and 164 control subjects were enrolled. CONCLUSION: The present study confirmed that CRS is frequent asthma comorbidity as concerns more than one-third of asthmatic outpatients. CRSwNP is associated with type 2 inflammation and blood eosinophilia. These outcomes underline that CRSwNPasthma phenotype deserves adequate attention for careful management and optimal identification of the best-tailored therapy. Conclusions Almost all patients with difficult-to-control asthma have comorbidities, in particular asthmatic women of older age, former smokers, and asthmatics who are prednisone dependent. Recognition of these typical characteristics can help physicians in the diagnostic workup, so that adequate preventive measures can be taken.

ConclusionVariability in antibiotic use in CRS appears to be driven by symptom burden, independent of objective measures of disease severity, patient demographics, and presence of comorbid disease. Clear guidelines are essential to define appropriate antibiotic use in CRS.
