Jpn J Infect Dis. 2013;66(2):83-8 Authors: Nakanaga K, Yotsu RR, Hoshino Y, Suzuki K, Makino M, Ishii N Abstract Buruli ulcer (BU) is an emerging human disease caused by Mycobacterium ulcerans, which mainly affects the extremities. It is most endemic in sub-Saharan Africa; however, it has been reported worldwide, including in some non-tropical areas. "M. ulcerans subsp. shinshuense" is proposed as a subspecies of M. ulcerans, which have been reported from Japan and China. A total of 35 BU cases have been reported as of November 2012. Although M. ulcerans is categorized as nontuberculous mycobacteria, it has some unique characteristics that could only be observed in this bacterium. It possesses a giant virulent plasmid, composed of 174-kbp nucleotides, coding polyketide synthase to produce macrolide toxin called mycolactone. The discovery of such a linkage of plasmid and its pathogenesis has not been reported in other human disease-causing mycobacteria. PMID: 23514902 [PubMed - in process]

Conclusion: The Buruli ulcer surveillance system is meeting its set objectives, and the data generated are used to reliably describe the epidemiologic situation and evaluate the results for actions and plan future interventions. We report here a case of Buruli ulcer in a patient who travelled in Senegal, a country not identified by the World Health Organization to be an endemic country for this disease. Our case is the third Buruli ulcer case reported as contracted in Senegal, showing that it is urgent to develop data collection in this country by having an active community-based surveillance-response system.

In this study, we detected M. ulcerans-specific sequences in rectal contents and spleens collected in wild grasscutters hunted in Buruli ulcer-endemic area in Côte d'Ivoire, but not in farmed negative control animals and in domesticated animals, namely, pigs, goats, cattle, and dogs, living in close contact with the local population. Some grasscutters exhibited the same sequence pattern in the feces and spleen. These observations confirm the asymptomatic gut carriage of M. ulcreans in this mammal species. ConclusionsBuruli ulcer should be included in the differential diagnosis of neonates who present with characteristic lesions. The incubation period of Buruli ulcer in neonates is probably shorter than is reported for adults.
