Issues with shoulder, neck, collar bone, back and chest.

Hi, I've always had poorly aligned hips and lower back pain, but in the last two years I began carrying around a very heaving backpack because of school. My back pain increased and the area spread from my lower back to my mid and upper back. However, this seems like a pretty obvious cause and result situation, and it's my own fault for not being more careful. My problem now is that my right shoulder has felt "wrong" for the past couple of months. I can't move it out of a slouch without using a hand to forcefully position it, and even if I do, it hurts and makes my entire arm tingle. It also makes a popping noise every time I try to move it on it's own, and it popped very loudly when I used my hand to push it back out of a slouch. Additionally, when I tried to un-slouch both of my shoulders and my upper back/neck for an extended period of time, I had difficulty breathing and my heart rate sped up, so I laid down because it's the only way I can not slouch. About a month ago, I went to the trainer at my high school and had him look at the shoulder because it had been very sore along with my collar bone. He said it seemed weak and was probably stretched, so he iced it for twenty minutes. It helped some with the pain, but the stiffness has remained and it feels like it's spread to my neck now, too, unless that's unrelated. I really love being active.
