Retrieval of a Migrated Cholecystectomy Clip
A 50-year-old woman with history of cholecystectomy and Roux-en-Y gastric bypass presented with acute exacerbation of chronic right upper quadrant pain and jaundice. Her serum bilirubin was 7 mg/dL, but she had no cholangitis. Computed tomography (CT) demonstrated dilation of the common bile duct (CBD) (Fig 1, arrow), an obstructing calculus, and a migrated cholecystectomy clip in the distal CBD (Fig 1, arrowhead). Percutaneous transhepatic cholangiography confirmed the distal obstructing calculus and the migrated cholecystectomy clip (Fig 2, arrow).
