ConclusionThe present study provides a corpus of knowledge considered essential for patients in the self-management of their arthritis. The selection of many items reflects recent emphasis on professional recommendations and the patients’ perspective. Future work should lead to the development of new updated KQs for patients with inflammatory arthritis.

Authors: Blazar PE, Gancarczyk SM, Simmons BP Abstract Since the advent of disease-modifying antirheumatic drugs for rheumatoid arthritis, orthopedic surgeons see fewer patients in the office who require hand surgery. However, a significant number of patients still seek surgical intervention to improve pain and function. These patients often present with isolated soft tissue pathologies, but even bone and joint pathology require meticulous soft tissue handling in this cohort. This review highlights the principles and techniques relevant to the management of soft tissue deformity in rheumatoid hand and wrist surgery...

However, both the exact origin and role of MΦs during inflammatory joint disease remain unclear. Here, we used multiple fate-mapping approaches in conjunction with 3D-light-sheet fluorescence microsco py and single cell RNA sequencing to perform a comprehensive spatiotemporal analysis of the compo...

Individuals and self-insured companies have adopted various strategies to reduce the rising cost of healthcare for themselves and their employees. A recent article discussed howWalmart was flying employees who were candidates for particular surgical procedures to selected, distant hospitals for evaluation (see:Walmart Flies Employees to Top Hospitals for Surgeries in a Bid to Cut Healthcare Costs). Below is an excerpt from it:Walmart ’s answer [to rising healthcare costs] is its six-year-oldCenters of Excellence (COE) program. In partnership with third-party administrator Health Design Plus (HDP), Walmart directly co...

AbstractBackground and aimsThough cyclooxygenase inhibitors are employed in rheumatoid arthritis treatment, modulators of leukotrienes are underexplored. We investigated the therapeutic potential of montelukast, a known cysteinyl leukotriene receptor-1 (CysLT1) inhibitor in an experimental rat model of arthritis.MethodsArthritis was induced in rats, and montelukast (5  mg/kg body wt.) was administered prophylactically (PAM) and therapeutically (TAM) through oral route. Results and discussionBlood and joint tissue markers of oxidative stress (lipid peroxidation, protein carbonyls, and nitric oxides) were significantly (p

AbstractThe predominance of the effector mechanisms by CD4  + T cells is a characteristic of inflammatory autoimmune diseases such as rheumatoid arthritis (RA). The CD40/CD40L costimulatory pathway contributes to these pathogenic mechanisms by promoting autoantibody production and inflammation. Aberrant expression of CD40 and CD40L in RA patients has be en shown, the latter prevailing in females. However, contrasting results have emerged regarding the clinical associations of these findings. We determined the association of CD40 and CD40L expression with the clinical activity evaluated through DAS28 in RA pat...

ConclusionsThere is limited experience of the design and implementation of trials for the prevention of inflammatory joint diseases. We discuss the rationale behind choice and duration of treatment and the challenges associated with defining the “at risk” state and offer pragmatic solutions in the protocol to enrolling subjects at risk of RA.Trial registrationCurrent Controlled Trials, ID:ISRCTN46017566. ConclusionUltrasound PD joint inflammation is associated with disease activity and is correlated with DAS28. In contrast, GS joint inflammation is associated with structural damage and is correlated with ultrasound ‐detected bone erosion.

Little is known on the outcomes of surgical aortic valve replacement (SAVR) versus transcatheter aortic valve implantation (TAVI) in patients with rheumatoid arthritis (RA).We queried the Nationwide Inpatient Sample Database (2012-2016). We performed a propensity-score-matched analysis based on 25 clinical and hospital variables to compare patients with RA who underwent SAVR versus TAVI. Our primary outcome was in-hospital mortality. Our final analysis included 5,640 hospitalizations with RA who underwent isolated AVR; of whom 2465 (43.7%) underwent TAVI.

This study included 200 consecutive adult RA patients. Clinical and laboratory investigations were performed. PROs including tender and swollen joint count, patient’s estimated pain, patient global assessment (PGA), validated Arabic version of the health assessment questionnaire (HAQ-A) as well as physician global assessment were considered. The disease activity score (DAS28) and simplified erosion narrowing score (SENS) were calculated. Patient health questionnaire-9 (PHQ-9) was used to detect depression.ResultsThe patients’ mean age was 41.3 ± 11.9 years and disease duration 6.6 ± 4.9...
