Late Breaking Abstract - Characteristics of macrophagal formula and cytokine spectrum of the bronchoalveolar lavage fluid in patients with newly diagnosed and those with recurring sarcoidosis of respiratory apparatus.

Materials and Methods: The bronchial examination with BALwas performed in 72 patients with newly diagnosed and recurrent sarcoidosis. The cytological investigation included the study of the lavage cytogram, calculation of the macrophage formula (MF) with examining of the percentage of young and mature macrophages with varies phenotypes, levels of the proinflammatory and anti-inflammatory cytokine.Results: In the newly diagnosed sarcoidosis, increase in the concentration of proinflammatory cytokines is observed in the BAL compared to the reference value of IL-2 (647.1 pg/ml versus 469), IL-8, IL-1β (2038.2 pg / ml vs. 920.2), IL-1β (424.15 pg / ml versus 267.8), the recurrent disease increase in the level of anti-inflammatory cytokines IL-4 (3798.0 pg/ml vs. 1084.5) and IL-5 (2006.0 pg/ml vs. 1454.6). In the newly diagnosed sarcoidosis, the content of macrophages with ultrastructural signs of secretory activity in BAL, is 8 times higher than normal value (26.6 %, normal 3.3 %). It correlates with the increased production of IL-8, IL-2, IL-1β and indicates the prevalence of the M1 type macrophages in lungs. In the recurrent sarcoidosis, the differentiation of young macrophages occurs either in the direction of development of their secretory or phagocytic function.
