Cerebral Vasoreactivity in Children with Sickle Cell Disease: A Transcranial Doppler Study

I read with great interest the original article by Macedo-Campos et al1 titled “Cerebral Vasoreactivity in Children with Sickle Cell Disease: A Transcranial Doppler Study.” Authors have explored the cerebrovascular reactivity assessed by breath-holding index (BHI) in the children with sickle cell disease (SCD). They have reported the impaired cerebrovascular reactivity in the children with SCD as compared to that of control subjects. However, few potential methodological limitations need to be addressed regarding this study.

This study was carried out to compare the fetomaternal outcome in patients with SCD with those without SCD. This was a retrospective cohort study done in the Eastern Province of Saudi Arabia in a tertiary care, teaching hospital, by retrieving the data through the code ICD-9 for SCD, the control group was also selected with comparable characteristics. Sickle cell disease (SCD) is the most commonly inherited genetic blood disorder, affecting nearly 100,000 children and adults in the United States. Three million Americans carry the sickle cell trait and it is nearly25 times more common in people of African American descent. The disorder, which can cause irregularly shaped blood cells to get stuck in narrow vessels, inflicts substantial pain on patients. The resulting reduced oxygen flow creates life-threatening complications such as difficulty breathing, chest pains, blood clots and stroke.

CONCLUSIONS: This review identified only nine studies, with insufficient data for all pharmacological interventions for analysis. The available evidence is very uncertain regarding the efficacy or harm from pharmacological interventions used to treat pain related to sickle cell VOC in adults. This area could benefit most from more high quality, certain evidence, as well as the establishment of suitable registries which record interventions and outcomes for this group of people.
