Hypovitaminosis D: comparison between patients with hip fracture and patients with vertebral fractures

This study analyses the difference in 25OH-vitamin D values between two groups of patients both affected by severe osteoporosis with fragility fractures, but one group has vertebral fractures and the other one has hip fractures. Patients with hip fractures have vitamin D values lower than patients with vertebral fractures.IntroductionThe purpose of this study was to evaluate 25OHD levels in patients with fragility vertebral fractures (VF) and hip fractures (HF) and make a comparison between the groups.MethodsIn the first group were enrolled ambulatory patients with 3 or more moderate to severe VF; in the second group were enrolled patients hospitalized in the Department of Orthogeriatrics undergoing surgery for HF. For all patients, we collected values of 25OHD and PTH. In conclusion, we found osteoblastogenesis was reduced in an acidic environment, and ASIC2, ASIC3, and ASIC4 were most highly expressed in turn during osteoblastogenesis within acidic media. ASIC2 was the most abundantly expressed gene in human bone cells in OVF compared with the control. ConclusionCare of spine fractures presenting acutely varies; a high proportion managed by ED solely. Whether outcomes vary as a result is not answered by this audit but there is a need for a pathway to inform best practice. Osteoporosis is inadequately-addressed in this high risk group, highlighting need for fracture liaison services in post-acute management.

Glucocorticoids-induced osteoporosis (GIOP) is a common form of secondary osteoporosis that often precipitates osteoporotic vertebral compression fracture (VCF). Comparing to conservative therapeutic strategies for VCF, percutaneous vertebroplasty (PVP) could quickly relieve back pain and improve overall life quality. But recent studies demonstrate that the risk of refracture following PVP surgery is high among patients with GIOPVCF. Thus, it is vital that the outcomes of different treatments for GIOPVCF are clearly understood and the clinical guidelines for treating this disease are defined.

Conclusion: Significant (90%) pain relief achieved within 24 to 48 hours in eighteen (81.81%) patients. Cath lab-PVP increases safety, early mobility without analgesics. Small volume PMMA is optimal and less prone for complication. Cath lab PVP fluoroscopy with its increased PMMA radio opacity monitors bone filling well.
