Persistent left superior vena cava associated with a circumflex coronary artery fistula

A 50-year-old man with long-standing atrial fibrillation with multiple failed ablations presented with recurrent arrhythmias. During evaluation for repeat ablation, transthoracic echocardiogram showed a dilated coronary sinus which opacified prior to the right atrium and ventricle following injection of agitated saline, suggestive of a persistent left superior vena cava (PLSVC).
