Hidden immune cells cause lung transplant failure

(Northwestern University) Scientists have discovered that a subset of immune cells called nonclassical monocytes, previously unknown to reside in the lungs, play a key role in driving primary graft dysfunction (PGD), the leading cause of death after lung transplantation. The study demonstrates targeting these cells could lead to novel treatments for PGD, a complication that currently impacts more than half of transplant patients.

In this report, early graft dysfunction was diagnosed at day 3 and the patient was empirically treated with antibiotics and high dose corticosteroid therapy. Death occurred on the 29th post-operative day and an autopsy revealed abundance of foamy macrophages in lung tissue consistent with ASMD cells.

Sepsis is a dynamic disease, displaying an inflammatory profile that varies over time and for each organ. The objective of our study was to evaluate the impact of CAS on lung function after lung transplantation. MATERIAL AND METHODS All lung transplant recipients from June 2009 to August 2014 in a single center (Strasbourg, France) were retrospectively reviewed. RESULTS A total of 191 lung transplantations were performed: 175 bilateral, 15 single, and 1 heart-lung transplantation. Of the 161 bilateral lung-transplanted patients who survived>3 months, 22 (13.6%) developed CAS requiring endobronchial treatment.
