This study investigates the safety and efficacy of a large-dose, needle-based epidural technique in obstetric anesthesia. The technique differs from a standard, catheter-based approach in that the anesthetic dose is administered through an epidural needle prior to insertion of the epidural catheter. Using a data-driven informatics and machine learning approach, our findings show that the needle-based technique is faster and more dose-effective in achieving sensory level. We also find that injecting large doses in the epidural space through the epidural needle is safe, with complication rates similar to those reported in pu...

Publication date: Available online 13 November 2019Source: Pharmacology Biochemistry and BehaviorAuthor(s): Taro Kato, Ronald S. DumanAbstractConventional antidepressant medications, which act on monoaminergic systems, have significant limitations, including a time lag of weeks to months and low rates of therapeutic efficacy. Recently, clinical findings demonstrate that ketamine, a dissociative anesthetic that blocks N-methyl-d-aspartate (NMDA) receptor channel activity, causes rapid (within hours) and long-lasting (7 to 10 days) antidepressant effects. Rapastinel is a novel glutamatergic compound that acts as an NMDAR p...

ConclusionsThis study shows that microsurgical lingual reconstructions are effective in patients with preoperative allodynia, but a small number of patients may have remaining allodynia after surgery. Large-scale, more detailed analysis of such cases is required.

This study evaluates the clinical effects of the stepwise anterior vitrectomy on the prevention of positive vitreous pressure (PVP) during penetrating keratoplasty (PKP).MethodsPKP in conjunction with stepwise anterior vitrectomy was performed on 15 eyes of 15 patients under retrobulbar anesthesia. A preset vitrectomy trocar-cannula was inserted into the vitreous cavity before PKP. During the opening of the anterior chamber, intermittent vitrectomy and corneal incision expansion were performed alternately to keep the lens or artificial intraocular lens (IOL) and iris flat until the entirety of the pathological cornea had b...

Publication date: Available online 14 November 2019Source: Surgery (Oxford)Author(s): Robert Crooks, Christopher JohnsonAbstractThe wide spectrum of neurological disorders and their effects on the function of the central and peripheral nervous system can cause an extensive array of symptoms and significant morbidity and mortality. This morbidity and mortality is often amplified in the perioperative period with an elevated risk of anaesthetic complications and adverse events. However, the number of patients with neurological disease requiring surgery is unlikely to diminish, and therefore this anaesthetic risk needs to be m...

I don't have much of an ego when it comes to these things but lately i've noticed more and more patients thinking anesthesiologists are not doctors. Comments like oh hes not the doctor hes just anesthesiologist. I'm sure they just mean he's not the surgeon but im beginning to wonder has the CRNAs brainwashing actually worked. Their plan has worked now I have to introduce myself as physician anesthesiologist...
