Conclusions: Although surgery remains a mainstay treatment for RHNC, intraoperative interstitial brachytherapy delivery as adjunctive therapy may improve the treatment outcome and may be associated with fewer complication rates as compared to reirradiation using external beam radiotherapy. High dose-rate interstitial brachytherapy (HDRBT) at our institution has been limited to an extremely selected group of patients with heterogeneous head and neck (HN) cancers due to significant concerns for its toxicity profile. Our HDRBT technique in HN cancers differs from that of many other institutions in that our treatments are planned based on CT images and optimized using inverse planning simulated annealing (IPSA), which allows us to deliver higher doses per fraction than most reports in the literature.
