Revascularizing Multivessel Coronary Artery Disease in Acute Coronary Syndromes: Choices and Questions

Acute coronary syndrome (ACS) is a leading cause of hospital admissions and mortality worldwide1. Revascularization of coronary artery disease (CAD) has been shown to improve cardiovascular outcomes including mortality in this patient population2. When a clear, single vessel culprit coronary artery lesion is identified on invasive coronary angiography, then the revascularization decision is straightforward with percutaneous coronary intervention (PCI) as the best option3. However, approximately 50% of patients presenting with ACS are found to have multivessel CAD, and then the decision on which revascularization strategy to choose becomes more complex and difficult4.
