Piloting a new care model for pain patients and other hospital super utilizers

To reduce unnecessary hospital utilization, a multidisciplinary team launched a pilot in October 2022 to provide high touch coordinated care to 10 inpatient super utilizers. Common features of this group include chronic pain (3), chronic opioid usage (3), and addiction (2). The 10 highest utilizers had a total of 190 hospitalizations and 264 ED visits in 12 months preceding the pilot. The goal is to demonstrate whether a new care model can improve complex patients ’ quality of life, reduce unnecessary hospital and ED utilization, and reduce total cost of care.
