Concomitant coronary artery disease (CAD) and carotid artery stenosis is a perennial clinical challenge. Determining the optimal medical or surgical management and subsequent timing of needed operative intervention remains elusive. Yu and Chaney describe a challenging case of symptomatic coronary and carotid artery disease which calls attention to the uncertainty in best practice.1 Although an abundance of retrospective literature exists on the subject, a consensus on one single best pathway does not exist and randomized controlled trials are limited.
