Functional mitral regurgitation and heart failure with preserved ejection fraction: clinical implications and management

Common pathophysiological mechanisms are observed in HFpEF and functional MR, such as diastolic dysfunction, preserved EF, atrial dilation and dysfunction, and annular remodeling. However, less is known about the prevalence and clinical value of functional MR in the context of HFpEF. The true prevalence of FMR has been overlooked for decades until a few years ago, and this has led to a significant underestimation of the diagnosis. This review aims to provide an overview of the known association between MR and HFpEF, discuss underlying mechanisms common to these diseases, and summarize potential targeted pharmacological treatments.
