ST Elevation is a poor surrogate for acute coronary occlusion. Let's Replace STEMI with Occlusion MI (OMI)!!

In this issue of the IJC, de Alencar Neto et al. publish a well done meta-analysis of all studies (total of 3) which report the sensitivity of ST Elevation (STE) criteria for angiographic Acute Coronary Occlusion MI (ACOMI, or OMI), and found that the pooled sensitivity was only 43.6%, in spite of the fact that it is generally taken for granted that STE is an adequate surrogate for OMI [1]. That the STEMI/NonSTEMI dichotomy has been in use for over 20  years without angiographic data on its validity is shocking.
