Outpatient Navigation Increases Enrollment In Serious Illness Services For Outpatients With Chronic Heart Failure

Heart failure is a common and costly medical illness with poor prognosis, with as low as 25% survival rate at 5 years post-index hospitalization. Many serious illness services (SIS) such as clinic-based palliative care, home based palliative care, hospice, and nurse care management have been developed to improve quality of life and reduce acute care utilization. However, these services remain underutilized. We evaluated how care navigation by a Serious Illness Navigator (SIN) impacts referral to and uptake of outpatient SIS and the effect on overall acute care utilization (ED visits and hospitalizations).
