Before Aortic Valve Replacement: Think Globally, Look Regionally

For a long time, cardiovascular imaging has aspired to provide rapid, accurate, and noninvasive evaluation of myocardial function. Being readily available, left ventricular ejection fraction (LVEF) remains the most frequently employed parameter in daily practice. Nevertheless, it has been demonstrated that the analysis of global longitudinal strain (GLS) using speckle-tracking analysis outperforms LVEF in the early detection of subclinical myocardial dysfunction and prediction of major events onset in various pathological situations, including aortic stenosis (AS),1-3 even though these 2 parameters are both dependent on the loading conditions.
