Chronic Crusted Plaque After Skin Injury

An 11-year-old male presented to the emergency department with a lesion on his right proximal lower leg. Two months earlier, he had fallen onto a branch, leading to a laceration requiring repair with sutures. This wound was improving initially; however, six weeks later, surrounding redness and overlying crusting developed. The child presented to a walk-in clinic and was started on topical mupirocin ointment and oral cephalexin for presumed cellulitis. The surrounding erythema worsened despite treatment with antibiotics, and he presented to the emergency department.
