Pneumopericardium: a Rare Complication of Marginal Ulcers

A 64 year old male with morbid obesity, status post Roux-en-y gastric bypass 14 years prior, presented to a community hospital with altered mental status. The patient's son reported that the patient had called him earlier in the day and endorsed feeling ill and short of breath. Physical exam revealed altered mental status and cool extremities. The patient was hypothermic with a temperature of 95.3 F. His blood pressure was 101/53 mmHg and heart rate was in the 80s. His respiratory rate and oxygen saturations were normal.
