Frequency of HLA-DR+CD38hi T cells identifies and quantifies T-cell activation in hemophagocytic lymphohistiocytosis, hyperinflammation, and immune regulatory disorders

Evaluation of T-cell activation is essential for the diagnosis of numerous hyperinflammatory and immune regulatory disorders, including hemophagocytic lymphohistiocytosis (HLH).1-6 Currently, plasma levels of soluble IL-2 receptor (sIL-2R) or sCD25/ αchain of the IL-2R complex are extensively used as a biomarker for measuring T-cell activation.7 Its role in the assessment of disease severity and monitoring has been established in a spectrum of hyperinflammatory disorders (HLH, multisystem inflammatory syndrome in children [MIS-C]),4,8,9 autoim mune disorders,10 infections (eg, HIV, EBV, coronavirus disease 2019 [COVID-19]),11,12 and malignancies (eg, non-Hodgkin lymphoma).
