Old People With Unipolar Depression on Seroquel 800mg
I inherited a dump of a case in the outpatient setting. 74 yo on Seroquel 800 (14 years), Trazodone 200, Prazosin 1mg AM, 2mg HS, Ativan 0.5 bid prn. Has an obviously high tolerance to the sedating effects of her regimen. Unipolar depression per the last treating psychiatrist of years, oh and a and a touch of ptsd. Why is trazodone rarely used for depression?
Why is trazodone rarely utilized as an early-on monotherapy option for depression? You would think it's a solid option with it's low incidence of weight gain and sexual side effects, combined with it's benefits on sleep. Maybe you run into tolerability issues when you dose it above the lower doses used for sleep? I'd be unaware personally, because I self-admittedly rarely use it like this as well. Switching patient from xanax to trazodone?
I have a patient who has been on xanax for years, been on 0.5 mg at bedtime for the past year. She states that it helps her with sleep, but she wants off the medication due to cognitive concerns and concerns for addiction. I told her to cut it to 0.25 mg at bedtime, with a plan to slowly switch to trazodone. Anyone have experience with this or advice? (Source: Student Doctor Network)

Anorgasmia Treatments?
Looking for some input from the hive mind here. 33yo with schizophrenia. Had him on inpt for about 4 months before he was finally stabilised on Abilify Maintena 400mg and olanzapine 10mg. But he’s had Anorgasmia since being on the meds. Not sure which one did it as he came to me on both. No erectile difficulties, he just can’t finish. I tried bupropion and mirtazapine, no effect. Adding Metformin to VPA to control weight gain?
I have a moderately obese gentleman with Schizoaffective disorder and Meth use history who is still having a lot of AH and mood instability on VPA 1,000 mg, Risperdal 8 mg, Trazodone 150 mg and Wellbutrin 150 mg BID (has been snorting at group home a few times). His BMI is 37 and he wants to switch to Zyprexa as it worked for him in the past. I told him I would need labs before making any changes. Trazodone-induced alopecia
I'm curious if anyone else is familiar with this. Obviously I was not aware of it until recently, and so far 3 other psychiatrists I have surveyed weren't either. I have a 12yo female on trazodone 25mg for sleep who lost all of the hair in both of her eyebrows about 3-4 weeks after starting. I looked it up and it's apparently a rare ADE, and the hair will more than likely grow back (unfortunately I can't find any actual quantification of this probability).
