poem
In the Conservatory with a CandlestickEvery moment is a lit matchFlickering in a dark roomSome gray February morningMenaced by strange winter thunder.Getting older is running out of matchesBurning each one down To the absolute nubSearing thumb and forefinger.Every iteration of old selfIs huddled somewhere in a quietEnglish conservatory Waiting to get murdered. All you hear is the sound of matches strikingAgainst strips of powdered glass Rapid at first and then spaced out In ever wider intervalsLike popcorn left too long in the microwave. It dusks and then gets darker and darkerAs everyone begins to ...

OnceTrees un-leavedNaked in the stillness Of frosted dawn,That old complacent wisdom —Winter passing, the coming of springBuds like curled fists.Birds returning to their nestsEvery winter we shiver A little bit longer.Hoarfrost whiskers our bones We slouch. We wither.Hips snap. We slip.Exhaust our listOf foolish gambits.Spring has passedAnd doesn ’t come back We bloom only onceThen hunt for blankets3/11/24 (Source: Buckeye Surgeon)

ConsultationThe patient came to see me for a consultation. He said his doctor had referred him. But he didn ’t know why. Just that it was critical that he see me as soon as possible. He wasn’t experiencing any pain. His weight had been steady. He denied fevers, sweats, rashes or catarrh. He hadn’t noticed any bulges, lesions, lumps or viscous secretions. He ate well and trimmed his nails. Drank only occasionally. Most mornings he woke at dawn and worked out. He seemed a paragon of robust middle age health. I had a busy day, many more patients to see and then a couple add-on cases after clinic, but I didn’t wa...

The Money ’ s in the Wrong Place. How to Fund Primary Care
By MATTHEW HOLT I was invited on the Health Tech Talk Show by Kat McDavitt and Lisa Bari and I kinda ranted (go to 37.16 here) about why we don’t have primary care, and where we should find the money to fix it. I finally got around to writing it up. It’s a rant but a rant with a point! We’re spending way too much money on stuff that is the wrong thing. 30 years ago, I was taught that we were going to have universal health care reform. And then we were going to have capitated at-risk entities. then below that, you have all these tech enabled services, which are going to make all this stuff work an...
