Silent cerebral infarcts in patients with atrial fibrillation: Clinical implications of an imaging-adjusted CHA2DS2-VASc score
CONCLUSIONS: Incorporation of silent infarcts into the CHA₂DS₂-VASc score may change the risk-benefit ratio of anticoagulation. It may also increase the number of patients who would benefit from periprocedural bridging. Future research should examine whether an anticoagulation strategy based on imaging-adjusted CHA₂DS₂-VASc scores could result in a greater reduction of stroke and cognitive decline.PMID:35703042 | DOI:10.5603/CJ.a2022.0055

Carotid Plaque Composition and the Importance of Non-Invasive in Imaging Stroke Prevention
Front Cardiovasc Med. 2022 May 16;9:885483. doi: 10.3389/fcvm.2022.885483. eCollection 2022.ABSTRACTLuminal stenosis has been the standard feature for the current management strategies in patients with atherosclerotic carotid disease. Histological and imaging studies show considerable differences between plaques with identical degrees of stenosis. They indicate that specific plaque characteristics like Intraplaque hemorrhage, Lipid Rich Necrotic Core, Plaque Inflammation, Thickness and Ulceration are responsible for the increased risk of ischemic events. Intraplaque hemorrhage is defined by the accumulation of blood compon...

Silent brain infarction is associated with carotid siphon calcification in ischemic stroke patients
CONCLUSIONS: Patients with SBI at acute phase of ischemic stroke have more risk factors than mentioned previously. SBI associated with the intimal pattern of CSC which relate to the atherosclerosis process in symptomatic ischemic stroke patients.PMID:35644109 | DOI:10.1016/j.nicl.2022.103050

Predicting LV thrombosis after acute myocardial infarction: From Virchow's triad to state-of-the-art cardiovascular magnetic resonance imaging
Despite the decline in incidence - in the era of primary percutaneous interventions (PCI), left ventricular (LV) thrombus remains a feared complication after acute myocardial infarction (AMI) due to the associated high risk of systemic embolism and stroke [1,2]. Depending on infarct/reperfusion-related characteristics as well as type and timing of diagnostic imaging modality, reported rates of LV thrombus following AMI are widely variable: from 2.7% for all and 9.1% for anterior ST-elevation myocardial infarction (STEMI), within 3  months on echocardiography, to 6.3% for all /19.2% for anterior STEMI with LV ejection fraction (LVEF)

Sex Differences in Intracranial Atherosclerosis in Patients With Hypertension With Acute Ischemic Stroke
Conclusions Men with hypertension with acute ischemic stroke have significantly higher total and proximal plaque burdens than women. Women with hypertension on anti-hypertensive medication showed a greater reduction in proximal plaque burden than men. Further confirmation with a longitudinal cohort study is needed and may help evaluate whether different treatment guidelines for managing hypertension by sex can help reduce intracranial atherosclerosis burden and ultimately acute ischemic stroke risk.PMID:35574965 | DOI:10.1161/JAHA.122.025579
