Guideline on carotid surgery for stroke prevention: updates from the Italian Society of Vascular and Endovascular Surgery. A trend towards personalized medicine
CONCLUSIONS: This GL updates on the main recommendations for the most appropriate diagnostic and medical-surgical management of patients with atherosclerotic carotid artery stenosis to prevent ischemic stroke. This GL also provides useful elements for the application of PM in good clinical practice.PMID:35848869 | DOI:10.23736/S0021-9509.22.12368-2

Modern Analysis of Stroke After Carotid Stenting in Octogenarians Using the Virginia Vascular Study Group Vascular Quality Initiative Data
Carotid artery stenting (CAS) is a safe and effective alternative to carotid endarterectomy in appropriate patients. However, there remains debate over the safety of stenting in octogenarians. This is due to previous studies demonstrating higher stroke risk in this population. Since those studies were published, transcarotid artery revascularization (TCAR) has become available, allowing surgeons to stent without crossing the aortic arch. Using a modern database, we sought to evaluate whether CAS is associated with increased odds of stroke in octogenarians compared to younger patients and to determine whether there is a dif...

Neurocritical Care Aspects of Ischemic Stroke Management
This article reviews the care of patients with ischemic stroke in the intensive care unit, including early general critical care interventions for airway control blood pressure goals according to the type of acute stroke treatment, poststroke cerebral edema management, hemorrhagic conversion in ischemic stroke, fibrinolytic reversal, and management of carotid endarterectomy and infective endocarditis. The importance of preventing common intensive care complications is discussed, including aspiration pneumonia, deep venous thrombosis, urinary tract infections, cardiac arrhythmias, and hyperglycemia.

Impact of presenting Stroke Severity and Thrombolysis on Outcomes After Urgent Carotid Interventions
Carotid interventions are increasingly performed in select patients after acute stroke. We aimed to determine the effects of presenting stroke severity (National Institutes of Health Stroke Scale [NIHSS]) and use of systemic thrombolysis (tPA) on clinical and discharge neurologic outcomes (modified Rankin scale [mRS] score) after urgent carotid endarterectomy (uCEA) and urgent carotid artery stenting (uCAS).

Impact of presenting stroke severity and thrombolysis on outcomes following urgent carotid interventions
Carotid interventions are increasingly performed in select patients following acute stroke. We aimed to determine the effects of presenting stroke severity (National Institutes of Health Stroke Scale [NIHSS]) and use of systemic thrombolysis (tissue plasminogen activator [tPA]) on discharge neurologic outcomes (modified Rankin scale [mRS]) following urgent carotid endarterectomy (uCEA) and urgent carotid artery stenting (uCAS).
