Successful Bridge From Va-Ecmo to Heart Transplant in an Adult after Mechanical Thrombectomy for Acute Ischemic Stroke
We present a unique case of a patient who underwent MT for acute ischemic stroke while on VA-ECMO with the resolution of neurological deficits and eventually underwent heart transplantation (HT).

Carotid Endarterectomy Prior to Ventricular Assist Device Placement in a Patient with Cardiogenic Shock
Atherosclerotic disease in the cervical vessels may be a contraindication for implantation of left ventricular assist devices (LVADs). The occurrence of ischemic stroke represents an obstacle to rehabilitation, and its incidence seems to be higher in this population. Carotid endarterectomy (CEA) is a preferable surgical treatment in patients with high bleeding risk. This case report suggests that CEA followed by implantation of LVADs can be feasible.

Improvement in Right Ventricular Stroke Work Index in Cardiogenic Shock Associated with Excellent Durable Left Ventricular Assist Implantation Results
Cardiogenic shock results from inadequate cardiac output to satisfy organ perfusion with potential need for pharmacologic and mechanical circulatory support for survival. Impella 5.0 insertion is used as a rescue device and bridge to durable mechanical circulatory support. We sought to evaluate changes in right ventricular parameters in patients bridged with Impella 5.0 undergoing durable left ventricular device implantation (LVAD).

Ischemic Stroke and Intracranial Hemorrhages during Percutaneous Left Ventricular Assist Device Cardiac Support
Mechanical circulatory support systems for cardiogenic shock and heart failure are associated with ischemic strokes and intracranial hemorrhages. Impella is a percutaneously placed, ventricular assist device for short-term cardiac support. We aimed to study the prevalence of acute neurologic complications during short-term support with Impella.

CONCLUSIONS: With the evolution of MCS, patient phenotype and outcomes are also changing over time. CF LVAD support is increasingly being used in the less ill patient phenotype and more patients are supported for destination therapy. Mean survival is now approaching 5 years, but adverse events, especially neurologic events, continue to have a detrimental impact on the success of CF LVAD support. Severe systemic vasospasm causing recurrent cardiac arrest after orthotopic heart transplantation
We report a case of refractory coronary and systemic vasospasm in a cardiac transplant recipient 10 years after transplantation, which ultimately required re-transplantation.
