Cerebral embolic protection during transcatheter heart interventions
EuroIntervention. 2023 Sep 18;19(7):549-570. doi: 10.4244/EIJ-D-23-00166.ABSTRACTStroke remains a devastating complication of transcatheter aortic valve replacement (TAVR), with the incidence of clinically apparent stroke seemingly fixed at around 3% despite TAVR's significant evolution during the past decade. Embolic showers of debris (calcium, atheroma, valve material, foreign material) are captured in the majority of patients who have TAVR using a filter-based cerebral embolic protection device (CEPD). Additionally, in systematic brain imaging studies, the majority of patients receiving TAVR exhibit new cerebral lesions...

Transcatheter aortic valve implantation versus surgical aortic valve replacement in patients at low to intermediate surgical risk: rationale and design of the randomised DEDICATE Trial
EuroIntervention. 2023 Sep 1:EIJ-D-23-00232. doi: 10.4244/EIJ-D-23-00232. Online ahead of print.ABSTRACTTranscatheter aortic valve implantation (TAVI) has become the preferred treatment option for patients with severe aortic stenosis at increased risk for surgical aortic valve replacement (SAVR) and for older patients irrespective of risk. However, in younger, low-risk patients for whom both therapeutic options, TAVI and SAVR, are applicable, the optimal treatment strategy remains controversial, as data on long-term outcomes remain limited. The DEDICATE-DZHK6 Trial is an investigator-initiated, industry-independent, prospe...

Long-term changes in coronary physiology after aortic valve replacement
Conclusions: CFR in the LAD increased significantly at follow-up although global hyperaemic flow and minimal microvascular resistance remained unchanged. Thus, a decrease in resting flow was the cause of CFR improvement. CFR improvement was associated with reduction in LVSW.PMID:36239118 | DOI:10.4244/EIJ-D-22-00621

Early outcomes of transcatheter mitral valve replacement with the Tendyne system in severe mitral annular calcification
CONCLUSIONS: In patients with MR and severe MAC, TMVR with the Tendyne valve was associated with encouraging acute outcomes, midterm durability, and clinical improvement. Dedicated TMVR therapy may have a future role in these anatomically challenging, high-risk patients.PMID:34918624 | DOI:10.4244/EIJ-D-21-00745
