Bleeding Issues During Thrombolysis for Acute Leg Ischaemia
When we started testing catheter directed low dose thrombolysis (CDT) for acute leg ischaemia (ALI) in the 1980s, it was the first time endovascular intervention advanced beyond simple balloon angioplasty. In many respects, CDT kick started the endovascular revolution. Clots could be removed without surgery, and from arteries too small for embolectomy catheters to reach. But there was a price to pay: thrombolytic drugs caused bleeding, mainly from catheter puncture sites, but also from distant areas, such as haemorrhagic stroke.

Isolated hand weakness due to brain lesion
Hand Surg Rehabil. 2022 Aug 12:S2468-1229(22)00172-4. doi: 10.1016/j.hansur.2022.08.003. Online ahead of print.ABSTRACTWhen hand weakness is linked to a brain lesion, there is risk of misdiagnosis. A case of hand weakness related to stroke is presented. The electromyography report indicated a segmental lesion of the median nerve at the wrist. Thus, the patient was initially diagnosed with carpal tunnel syndrome, and was referred to the orthopedic department for release surgery. Because the symptom was immediate and did not include hand numbness, the orthopedic surgeon ruled out the previous diagnosis and ordered a head CT ...

Peri-procedural variables and outcomes of long-period hemodynamic instability after carotid artery angioplasty and stenting
CONCLUSIONS: CAS-induced HI occurs in a considerable percentage while several peri-procedural variables are determined as independent predictors to develop long-period HI. Patients with prolonged HI are associated with increased risk of neurologic events and thus standardized intervention as well as management of long-period HI are of critical importance during clinical process.PMID:35466838 | DOI:10.1177/17085381221091369

Immediate and Long Term Results of a Randomised Single Centre Study of Surgical Treatment of Asymptomatic Stenosis of the Internal Carotid Arteries
Introduction: Cerebrovascular accident (CVA) is one of the main causes of disability and mortality in many developed countries of the world. The results of numerous studies have confirmed the positive effect of carotid endarterectomy (CEA) in the prevention of stroke development in both symptomatic and asymptomatic patients. Balloon angioplasty (BA) and carotid artery stenting (CAS) have evolved, in part, because of the need for alternative, less invasive treatments for patients considered poor candidates for surgery based on anatomical or clinical parameters.

Chimney Endovascular Aneurysm Repair From Below is Attractive but Carries the Risk of a Relevant Complication
For the placement of chimney grafts, upper extremity access is necessary however, not all patients have suitable aortic arch anatomy, and it also carries a risk of stroke. In this context, successful chimney endovascular aneurysm repair (ChEVAR) via femoral access is attractive. Seven years ago, we presented the lift technique, which we used in 13 patients, placing self expanding covered stents approximately 1 –2 cm into the target renal artery from below. After fixation of the distal end with an angioplasty balloon, an 8 F sheath was carefully pushed over a stiff guidewire, lifting the chimney endograft upwards.

Evaluation of mortality following paclitaxel drug-coated balloon angioplasty of femoropopliteal lesions in patients with ulcerations and gangrene - a single center experience.
Conclusions: This monocentric retrospective analysis showed no survival disadvantage for patients in Rutherford-Becker stage 5 after treatment with paclitaxel-coated balloons. PMID: 33118425 [PubMed - as supplied by publisher]

Atherectomy with balloon angioplasty compared to balloon angioplasty alone for the treatment of chronic limb threatening ischemia: A national surgical quality improvement program database analysis.
CONCLUSION: Atherectomy with balloon angioplasty of femoropopliteal disease provides a significant decrease in untreated loss of patency compared to balloon angioplasty alone. PMID: 33045944 [PubMed - in process]

PC164 Fast-Track Thrombolysis for Acute Lower Extremity In-Stent Occlusions: A Novel Approach to Minimize Complications of Standard Thrombolytic Therapy
The role of catheter-directed thrombolysis (CDT) in the treatment of acute lower extremity ischemia may require prolonged periods of time to achieve successful lysis. Prolonged thrombolysis infusion has demonstrated increased incidence of intracranial bleeding, stroke, and local complications. It is expensive and increases hospital length of stay. To minimize these potentially negative outcomes, we developed a fast track approach (FTA) that included the use of aggressive balloon angioplasty and stenting before the thrombus was completely lysed.

Innominate Artery Stenosis Presenting With Migratory Digital Ischemia
Conclusion: Migratory digital ischemia is a rare presentation of atherosclerotic disease of the supra-aortic vessels. It represents a challenging situation for endovascular intervention due to concern about plaque dislodgment during the procedure. A retrograde balloon-stent angioplasty following distal carotid control with an open approach may be safe and effective in selected patients.

Combined carotid endarterectomy and retrograde stenting of the supra-aortic trunk: Does cervical block offers advantages?
Atherosclerosis of the carotid artery is a major source of stroke. In some cases atherosclerosis occurs at several positions within the carotid artery. Carotid endarterectomy (CEA) in combination with retrograde balloon angioplasty and stenting of a brachiocephalic or common carotid artery stenosis has been described as efficacious and save procedure to prevent stroke in these cases. The aim of this study was to analyse the impact of anaesthetic techniques on hemodynamic factors, operation time, duration of clamping and post-operative pain.

Impact of Perioperative Complications After Endovascular Therapy in Diabetic Patients With Critical Limb Ischemia due to Isolated Infrapopliteal Lesions
Conclusion: POCs within 30 days after balloon angioplasty for infrapopliteal disease in diabetics with CLI were more likely to occur in patients with low BMI and tissue loss. POC occurrence was associated with major adverse limb events in follow-up but not with mortality or wound healing.

Regarding “Poststent ballooning is associated with increased periprocedural stroke and death rate in carotid artery stenting”
We read with interest this article regarding the risks of balloon angioplasty during carotid stenting procedures. We applaud the authors for recognizing that the goal of carotid stenosis treatment should be stabilization of plaque emboli, not restoration of normal vessel diameter. We would like to point out that there is evidence that carotid stenting can be safely and effectively performed without the use of prestent or poststent ballooning.1,2 As the authors acknowledge, self-expanding stents continue to expand after deployment.
