Association of education and feedback on hypertension management with risk for stroke and cardiovascular disease
CONCLUSIONS: This study does not support an association between education and feedback on hypertension management to primary care physicians and the risk for stroke or cardiovascular outcomes. The observed differences for mortality outcomes should be interpreted with caution.PMID:35179089 | DOI:10.1080/08037051.2022.2041393

Stroke prophylaxis in critically-ill patients with new-onset atrial fibrillation
AbstractDespite a high incidence of new onset atrial fibrillation (NOAF) in critically ill patients and its association with short and long-term incidence of stroke, there is limited data assessing anticoagulation on hospital discharge in these patients. We retrospectively reviewed electronic medical records of all adult patients admitted to non-cardiac ICUs at our institution between January 2009 and March 2016. Patients with NOAF were identified and CHA2DS2-VASc score of ICU survivors was calculated. Prescription of oral anticoagulant therapy on hospital discharge was analyzed. A total of 640 (1.7% [38,708 patients]; 95%...

Prognostic impact of increased pulse pressure/stroke index in a registry of hypertensive patients: the Campania Salute Network.
CONCLUSIONS: In treated hypertensive patients, high PP/SVi predicted increased rate of MACE, independent of common confounders. PMID: 31068016 [PubMed - as supplied by publisher]

Stroke and Thromboembolism in Patients with Heart Failure and Sinus Rhythm: A Matter of Risk Stratification?
Thromb Haemost. 2022 Jan 19. doi: 10.1055/a-1745-2083. Online ahead of print.ABSTRACTPatients with heart failure (HF) in sinus rhythm (SR) experience an increased incidence of thromboembolic events including stroke. Among patients with HF, high-quality evidence supports the use of oral anticoagulation when atrial fibrillation (AF) is present, but the benefit of anticoagulation in SR in absence of other known indications for anticoagulation is unclear. In four randomized controlled trials (RCTs), warfarin did not improve a composite of clinical outcomes compared with aspirin or placebo in patients with HF with reduced eject...

Traditional risk factors and combined genetic markers of recurrent ischemic stroke in adults
CONCLUSIONS: Our findings suggest that cumulative effect of both traditional and common genetic risk factors were associated with recurrence of ischemic stroke. We demonstrated for the first time that a combined genotype FVL/MTHFR profile increase the risk of a second cerebral ischemic attack.PMID:34241942 | DOI:10.1111/jth.15448

Early rhythm control and the risks of ischaemic stroke, heart failure, mortality and adverse events when performed early ( & lt;3 months)
CONCLUSIONS: Early rhythm control, especially when performed earlier (<3months), was associated with a lower risk of adverse events than usual care among patients with early AF.PMID:35322396 | DOI:10.1055/a-1807-0336

No evidence for a J-shaped curve in treated hypertensive patients with increased cardiovascular risk: The VALUE trial.
In conclusion, patients in BP strata ≥ 150/90 mmHg, but not patients in BP strata < 130/70 mmHg, were at increased risk for adverse outcomes in this hypertensive, high-risk population. Although benefit in preventing MI in relation to preventing stroke levels off for the lowest BPs, these data provide no support for a J-curve in the treatment of high-risk hypertensive patients . The increase in the ratio of MI to stroke with lower DBP indicates target organ heterogeneity in that the optimal on-treatment DBP for cerebroprotection is below that for cardioprotection. Cessation of oral anticoagulation in relation to mortality and the risk of thrombotic events in patients with atrial fibrillation.
In conclusion, in AF patients AF, VKA cessation is independently associated with mortality stroke and cardiovascular events. Specifically, VKA cessation independently increased the risk of stroke, even after adjusting for CHA2DS2-VASc score. TTR was an independent risk factor for major bleeding following initiation of VKA therapy. PMID: 24096615 [PubMed - as supplied by publisher]
