Mr. M
Dr. Migliore I step off the subway, up the elevator, and up several flights of stairs. There’s a homeless man there who always uses the last stair of the lower staircase as a tray for his food. And by tray, I mean he sets his half-eaten rice burrito directly onto the step and picks around at it while I imagine the guacamole is mixing with Hep B and C. diff and MRSA. He looks up at me, I promise myself for the 100th time I’m going to start carrying cash or Kind bars or something; I give him my best sheepish face, and emerge at street level. I walk toward my hospital and pass a gentleman who sits on a ledge next to the ...

Zika associated Guillain-Barre syndrome in the United States (P2.327)
Conclusions:Zika can induce GBS after a brief viral illness. Neuropathy can be a mixture of demyelinating and axonal. Dysautonomia may also be present including severe dysphagia, orthostatics and retention.Zika RNA has a short detection window in serum and CSF making a diagnosis time sensitive. Serological assays using IgM and IgG cross-react with other flaviviruses making them unable to differentiate between recent Zika exposure and cross reactivity with Dengue and Chikungunya. Our pt likely had prior exposure to Dengue, being from an endemic area. Prior Dengue exposure may possibly modulate the response to Zika via antib...

Sequencing of Linkage Region on Chromosome 12p11 Identifies PKP2 as a Candidate Gene for Left Ventricular Mass in Dominican Families
In conclusion, sequencing of our previously reported QTL identified common and rare variants within PKP2 to be associated with LVM. Future studies are necessary to elucidate the role these variants play in influencing LVM.

Intrinsic capacity and its associations with incident dependence and mortality in 10/66 Dementia Research Group studies in Latin America, India, and China: A population-based cohort study
ConclusionsIn this study we observed a high prevalence of DICs, particularly in older age groups. Those affected had substantially increased risks of dependence and death. Most needs for care arose in those with DIC yet to become frail. Our findings provide some support for the strategy of optimising intrinsic capacity in pursuit of healthy ageing. Implementation at scale requires community-based screening and assessment, and a stepped-care intervention approach, with redefined roles for community healthcare workers and efforts to engage, train, and support them in these tasks. ICOPE might be usefully integrated into commu...
