Range of Risk Factor Levels: Control, Mortality, and Cardiovascular Outcomes in Type 1 Diabetes Mellitus.
CONCLUSIONS: A steep-graded association exists between decreasing number of cardiovascular risk factors at target and major adverse cardiovascular outcomes among patients with T1DM. However, risks for all outcomes were numerically higher for patients with T1DM compared with controls, even when all risk factors were at target, with risk for acute myocardial infarction and heart failure hospitalization statistically significantly higher. The Use of Biomarkers to Predict Specific Causes of Death in Patients with Atrial Fibrillation: Insights from the ARISTOTLE Trial.
Conclusions -Biomarkers were some of the strongest predictors of cause-specific death and may improve the ability to discriminate among patients' risks for different causes of death. These data suggest a potential role of biomarkers for identification of patients at risk for different causes of death in patients anticoagulated for AF. Assessment of Cardiovascular Risk in Older Patients with Gout Initiating Febuxostat versus Allopurinol: A Population-Based Cohort Study.
Conclusions -Among a cohort of 99,744 older Medicare patients with gout, overall there was no difference in the risk of MI, stroke, new onset HF, coronary revascularization, or all-cause mortality between patients initiating febuxostat compared with allopurinol. However, there seemed to be a trend toward an increased, albeit not statistically significant, risk for all-cause mortality in patients who used febuxostat for over 3 years versus allopurinol for over 3 years. The risk of HF exacerbation was slightly lower in febuxostat initiators. PMID: 29899013 [PubMed - as supplied by publisher]

Aromatase Inhibitors and the Risk of Cardiovascular Outcomes in Women With Breast Cancer: A Population-Based Cohort Study.
CONCLUSIONS: In this population-based study, aromatase inhibitors were associated with increased risks of heart failure and cardiovascular mortality compared with tamoxifen. There were also trends toward increased risks, although nonsignificant, of myocardial infarction and ischemic stroke. The increased risk of cardiovascular events associated with aromatase inhibitors should be balanced with their favorable clinical benefits compared with tamoxifen. The COMPASS Trial: Net Clinical Benefit of Low-Dose Rivaroxaban Plus Aspirin as Compared to Aspirin in Patients Chronic Vascular Disease.
Conclusions: Compared to ASA monotherapy the combination of rivaroxaban 2.5mg twice daily+ ASA resulted in fewer NCB events primarily by preventing adverse efficacy events, particularly stroke and cardiovascular mortality, whereas severe bleedings were less frequent and with less clinical impact. The NCB was particularly favorable in high-risk subgroups and those with multiple risk characteristics. Clinical Trial Registration: URL: https://www.clinicaltrials.gov. Research Priorities in Atrial Fibrillation Screening: A Report From a National Heart, Lung, and Blood Institute Virtual Workshop.
khia MP, Van Gelder IC, Cooper LS, Al-Khatib SM Abstract Clinically recognized atrial fibrillation (AF) is associated with higher risk of complications, including ischemic stroke, cognitive decline, heart failure, myocardial infarction, and death. It is increasingly recognized that AF frequently is undetected until complications such as stroke or heart failure occur. Hence, the public and clinicians have an intense interest in detecting AF earlier. However, the most appropriate strategies to detect undiagnosed AF (sometimes referred to as subclinical AF) and the prognostic and therapeutic implications of AF detect...

Characteristics of Heart Failure With Preserved Ejection Fraction Across the Range of Left Ventricular Ejection Fraction
CONCLUSIONS: Patients with HFpEF in whom LVEF ranged from 50% to 60% demonstrated reduced contractility, impaired ventriculo-arterial coupling, and higher extracellular volume fraction. In contrast, patients with HFpEF and a LVEF >60% demonstrated a hypercontractile state with excessive LV afterload and diminished preload reserve. A LVEF-based stratification of patients with HFpEF identified distinct morphologic and pathophysiologic subphenotypes.PMID:35862208 | DOI:10.1161/CIRCULATIONAHA.122.059280

Causes of Death and Influencing Factors in Patients with Atrial Fibrillation: A Competing Risk Analysis from the Randomized Evaluation of Long-Term Anticoagulant Therapy Study.
CONCLUSIONS: The majority of deaths are not related to stroke in a contemporary anticoagulated AF population. These results emphasize the need to identify interventions beyond effective anticoagulation, in order to further reduce mortality in AF. Long-Term Coffee Consumption and Risk of Cardiovascular Disease: A Systematic Review and a Dose-Response Meta-Analysis of Prospective Cohort Studies.
CONCLUSIONS: A non-linear association between coffee consumption with CVD risk was observed in this meta-analysis. Moderate coffee consumption was inversely significantly associated with CVD risk, with the lowest CVD risk at 3 to 5 cups/d, and heavy coffee consumption was not associated with elevated CVD risk. Long-Term Outcomes of Secondary Atrial Fibrillation in the Community: The Framingham Heart Study.
CONCLUSIONS: -AF recurs in most individuals, including those diagnosed with secondary precipitants. Long-term AF-related stroke and mortality risks were similar between individuals with and without secondary AF precipitants. Future studies may determine whether increased arrhythmia surveillance or adherence to general AF management principles in patients with reversible AF precipitants will reduce morbidity. Fructosamine and Glycated Albumin and the Risk of Cardiovascular Outcomes and Death.
CONCLUSIONS: -The acceptance of new measures of hyperglycemia is partly dependent on establishing their association with long-term outcomes. We found that fructosamine and glycated albumin were associated with vascular outcomes and mortality and that these associations were similar to those observed for HbA1c. Relation of Smoking with Total Mortality and Cardiovascular Events Among Patients with Diabetes: A Meta-Analysis and Systematic Review.
CONCLUSIONS: -Active smoking is associated with significantly increased risks of total mortality and cardiovascular events among diabetic patients, while smoking cessation was associated with reduced risks compared to current smoking. The findings provide strong evidence for the recommendation of quitting smoking among diabetic patients. PMID: 26311724 [PubMed - as supplied by publisher]

Retinal Vessel Calibers in Predicting Long-term Cardiovascular Outcomes: The Atherosclerosis Risk in Communities Study.
CONCLUSIONS: -Narrower retinal arterioles and wider retinal venules conferred long-term risk of mortality and ischemic stroke in both genders and CHD in women. These measures serve as an inexpensive, reproducible biomarker that added incremental value to current practice guidelines in ASCVE risk prediction in low-risk women.
