Cardiovascular Disease Among Persons Living With HIV: New Insights Into Pathogenesis and Clinical Manifestations in a Global Context
Circulation. 2023 Jan 3;147(1):83-100. doi: 10.1161/CIRCULATIONAHA.122.057443. Epub 2022 Dec 28.ABSTRACTWidespread use of contemporary antiretroviral therapy globally has transformed HIV disease into a chronic illness associated with excess risk for disorders of the heart and circulatory system. Current clinical care and research has focused on improving HIV-related cardiovascular disease outcomes, survival, and quality of life. In high-income countries, emphasis on prevention of atherosclerotic coronary artery disease over the past decade, including aggressive management of traditional risk factors and earlier initiation ...

Elevated blood pressure in pregnancy and subsequent chronic disease risk.
CONCLUSIONS: Elevated blood pressure during pregnancy, regardless of type and even without known risk factors, signals high risk of later cardiovascular disease, chronic kidney disease, and diabetes mellitus. Clinical monitoring, risk factor evaluation, and early intervention could benefit women with hypertension in pregnancy. PMID: 23401113 [PubMed - in process]

Riociguat for Patients with Pulmonary Hypertension due to Systolic Left Ventricular Dysfunction: A Phase IIb Double-Blind, Randomized, Placebo-Controlled, Dose-Ranging Hemodynamic Study.
CONCLUSIONS: Although the primary endpoint of the study was not met, riociguat was well tolerated in patients with PH-sLVD and improved cardiac index, and pulmonary and systemic vascular resistance. Predictors of Long Term Outcomes in Symptomatic Hypertrophic Obstructive Cardiomyopathy Patients Undergoing Surgical Relief of Left Ventricular Outflow Tract Obstruction.
CONCLUSIONS: Symptomatic adult HCM patients undergoing surgery for relief of LVOT obstruction have low event rate during long-term follow up; with worse outcomes predicted by increasing age and presence of residual AF during follow up. PMID: 23770748 [PubMed - as supplied by publisher]

New Oral Anticoagulants in Atrial Fibrillation Forever?
Abstract Atrial fibrillation is the most common chronic cardiac arrhythmia in clinical cardiology. It affects about 1% of the population, and of individuals over the age of 80 about 10% have this rhythm disturbance. Due to the loss of the atrial contribution to ventricular filling, left ventricular function is diminished resulting in a propensity to heart failure, fatigue and disability. Furthermore, the sensation of palpations can be very disturbing for younger patients and may hamper them in their physical and professional activities. Finally, the diminished blood flow through the heart, especially the left atri...

Outcomes of Urgent Warfarin Reversal Using Fresh Frozen Plasma versus Prothrombin Complex Concentrate in the Emergency Department.
CONCLUSIONS: Octaplex for urgent reversal of warfarin resulted in faster reversal, lower red cell transfusion requirements with fewer adverse events than frozen plasma. PMID: 23770745 [PubMed - as supplied by publisher]

Plasminogen Activator Inhibitor-1: A Novel Therapeutic Target for Hypertension?
Abstract Hypertension is the most common reversible risk factor for stroke, myocardial infarction, and heart failure, affecting greater than 60 million individuals in the United State alone and 1 billion globally(1). According to the World Health Organization, hypertension is now the leading cause of preventable death worldwide(2). Importantly, the prevalence and complications of hypertension increase significantly with age. However, despite the availability of many anti-hypertensive agents of distinct pharmacologic classes and evidence-based guideline recommendations for step-wise, multi-drug regimens, control of...

Using Benefit-Based Tailored Treatment to Improve the Use of Antihypertensive Medications.
CONCLUSIONS: We found that benefit-based tailored treatment was both more effective and required less antihypertensive medication than current guidelines based on treating to specific blood pressure goals. PMID: 24190955 [PubMed - as supplied by publisher]

Predictors of Long-Term Outcomes in Patients with Significant Myxomatous Mitral Regurgitation Undergoing Exercise Echocardiography.
CONCLUSIONS: In patients with ≥III+ myxomatous MR undergoing ExEc, lower %age-gender predicted METS, lower HRR, atrial fibrillation, lower LV ejection fraction and high resting RVSP predicted worse outcomes. PMID: 24396041 [PubMed - as supplied by publisher]

Risk of End-Stage Renal Disease and Death after Cardiovascular Events in Chronic Kidney Disease.
CONCLUSIONS: -CV events are common in patients with CKD stages 3-5 and are associated with a substantial increase in the risk of ESRD and ACM prior to ESRD. Intensive primary and secondary prevention strategies may help attenuate this risk. PMID: 24899688 [PubMed - as supplied by publisher]

Effects of Short-Term Continuous Positive Airway Pressure on Myocardial Sympathetic Nerve Function and Energetics in Patients with Heart Failure and Obstructive Sleep Apnea: A Randomized Study.
CONCLUSIONS: -In patients with HFrEF and OSA, short-term CPAP increased HED retention, indicating improved myocardial SN function, but overall did not affect energetics. In those with more severe OSA, CPAP may improve cardiac efficiency. Further outcome-based investigation of the consequences of CPAP is warranted. Reversible De Novo Left Ventricular Trabeculations in Pregnant Women: Implications for the Diagnosis of Left Ventricular Non-Compaction in Low Risk Populations.
CONCLUSIONS: -Pregnancy induces de novo LV trabeculations in a significant proportion of women. The results suggest that left ventricular trabeculations occur in response to increased LV loading conditions or other physiologic responses to pregnancy and are not specific for LVNC. These factors should be considered when assessing individuals with LV trabeculations outside the context of symptoms of heart failure or familial cardiomyopathy. Heart Failure, Saxagliptin and Diabetes Mellitus: Observations from the SAVOR - TIMI 53 Randomized Trial.
CONCLUSIONS: -In the context of balanced primary and secondary endpoints, saxagliptin treatment was associated with an increased risk for hospitalization for heart failure. This increase in risk was highest among patients with elevated levels of natriuretic peptides, prior heart failure, or chronic kidney disease.
