Noncardiac comorbidities and intensive up ‐titration of oral treatment in patients recently hospitalized for heart failure: insights from the STRONG‐HF trial

Impact of comorbidities on health status measured using the Kansas City Cardiomyopathy Questionnaire in patients with HFrEF and HFpEF
ConclusionsCardiac and non-cardiac comorbidities are common in both HFrEF and HFpEF patients and most are associated with reductions in health status although the impact varied among comorbidities, by the number of comorbidities, and by HF phenotype. Influenza vaccination and cardiovascular events in patients with ischaemic heart disease and heart failure: A meta ‐analysis
ConclusionIn this meta-analysis of available RCTs in patients at high cardiovascular risk, influenza vaccination was associated with a reduced incidence of cardiovascular events, cardiovascular death and all-cause death as compared to placebo or no treatment.

Impact of multimorbidity on mortality in heart failure with reduced ejection fraction: which comorbidities matter most? An analysis of PARADIGM ‐HF and ATMOSPHERE
ConclusionsIn HF, the impact of multimorbidity differed at the individual patient and population level, depending on the prevalence of and the risk related to each comorbidity, and the interaction between individual comorbidities. Patients with coexistent PAD and stroke were at greatest individual risk whereas, from a population perspective, coexistent CKD and hypertension mattered most.

Ventricular Stiffening and Chamber Contracture in Heart Failure with Higher Ejection Fraction
ConclusionMost pathophysiologic differences in patients with HFpEF and higher EF are related to smaller heart size, increased LV diastolic stiffness, and leftward shift in the EDPVR. These findings may help to explain the absence of efficacy for neurohormonal antagonists in this group and raise a new hypothesis, that interventions to stimulate eccentric LV remodeling and enhance diastolic capacitance may be beneficial for patients with HFpEF and EF in the higher range.

Effects of haemodynamically atrio ‐ventricular optimized His bundle pacing on heart failure symptoms and exercise capacity: the His Optimized Pacing Evaluated for Heart Failure (HOPE‐HF) randomized, double‐blind, cross‐over trial
ConclusionHis bundle pacing did not increase peak oxygen uptake but, under double-blind conditions, significantly improved quality of life and was symptomatically preferred by the clear majority of patients. Ventricular pacing delivered via the His bundle did not adversely impact ventricular function during the 6  months.

Effect of Patisiran on Stroke Volume in Hereditary Transthyretin ‐Mediated Amyloidosis: Insights from Pressure‐Volume Analysis of the APOLLO Study
ConclusionsPatisiran may delay progression of LV chamber dysfunction starting at 9 months of therapy. These data elucidate the mechanisms by which transthyretin reducing therapies modulate progression of cardiac disease and need to be confirmed in ongoing phase III trials.

Normal and excessive muscle sympathetic nerve activity in heart failure: implications for future trials of therapeutic autonomic modulation
ConclusionBurst frequency and incidence exceeded normative values in only ~53% and ~33% of patients. Such diversity encourages selective deployment of sympatho-modulatory therapies. Clinical characteristics can highlight individuals who may benefit from future personalized interventions targeting pathological sympathetic activation.

The Effects of Mineralocorticoid Receptor Antagonists on Cardiovascular Outcomes in Patients With End ‐Stage Renal Disease and Heart Failure
ConclusionsIn patients undergoing regular dialysis who are diagnosed with heart failure, the use of MRAs is associated with lower risks of all-cause mortality and CV death. The benefits of MRA treatment in heart failure may persist in patients with ESRD.
