Medical costs for managing chronic kidney disease and related complications in patients with chronic kidney disease and type 2 diabetes
CONCLUSIONS: Management of CKD and its complications incurs high medical costs for patients with CKD and T2D. Results from this study can be used to quantify the economic profile of emerging treatments and inform decision-making.PMID:34878754 | DOI:10.37765/ajmc.2021.88807

COVID-19 Exposed the Faults in America ’s Elder Care System. This Is Our Best Shot to Fix Them
For the American public, one of the first signs of the COVID-19 pandemic to come was a tragedy at a nursing home near Seattle. On Feb. 29, 2020, officials from the U.S. Centers for Disease Control and Prevention (CDC) and Washington State announced the U.S. had its first outbreak of the novel coronavirus. Three people in the area had tested positive the day before; two of them were associated with Life Care Center of Kirkland, and officials expected more to follow soon. When asked what steps the nursing home could take to control the spread, Dr. Jeff Duchin, health officer for Seattle and King County, said he was working w...

Hypertrophic cardiomyopathy considerations for the managed care pharmacist
Am J Manag Care. 2021 Apr;27(6 Suppl):S118-S125. doi: 10.37765/ajmc.2021.88629.ABSTRACTHypertrophic cardiomyopathy (HCM) is often seen in patients as an autosomal dominant genetic heart disease with a variable clinical course. It is characterized by left ventricular hypertrophy, and with some patients, there is no evidence of a genetic etiology or presence of HCM in family members. Young age at diagnosis and the presence of a pathogenic or likely pathogenic sarcomere variant predict greater lifelong risk for stroke, heart failure, ventricular arrhythmia, atrial fibrillation, or mortality. Most individuals affected with HCM...

Cardiorespiratory coupling in cetaceans; a physiological strategy to improve gas exchange? RESEARCH ARTICLE
Andreas Fahlman, Stefan Miedler, Luis Marti-Bonmati, Diana Ferrero Fernandez, Paola Munoz Caballero, Julietta Arenarez, Julie Rocho-Levine, Todd Robeck, and Ashley Blawas In the current study we used transthoracic echocardiography to measure stroke volume (SV), heart rate (fH) and cardiac output (CO) in adult bottlenose dolphins (Tursiops truncatus), a male beluga whale calf [Delphinapterus leucas, body mass (Mb) range: 151–175 kg] and an adult female false killer whale (Pseudorca crassidens, estimated Mb: 500–550 kg) housed in managed care. We also recorded continuous electrocardiogram (ECG) in the ...

Costs associated with renal and cardiovascular events among patients with type 2 diabetes mellitus and nephropathy: a cost model based on the CREDENCE clinical trial.
Conclusions: Positive costs avoided for each of the outcomes considered were predicted in the main analysis, with ESKD as the outcome predicted to have the greatest costs avoided at $2.92 PMPY. PMID: 31916465 [PubMed - as supplied by publisher]

Two New Drugs Help Relieve Sickle-Cell Disease. But Who Will Pay?

Sickle cell disease: current treatment and emerging therapies.
Abstract Sickle cell disease (SCD) is among the most common genetic diseases in the United States, affecting approximately 100,000 people. In the United States, SCD is characterized by a shortened life expectancy of only about 50 years in severe subtypes, significant quality-of-life impairments, and increased healthcare utilization and spending. SCD is characterized by chronic hemolytic anemia, vaso-occlusion, and progressive vascular injury affecting multiple organ systems. The pathophysiology is directly related to polymerization of deoxygenated hemoglobin, leading to a cascade of pathologic events including ery...

Medicare's bundled payment model did not change skilled nursing facility discharge patterns.
CONCLUSIONS: Hospital participation in BPCI was not associated with changes in the number of SNF partners or in discharge concentration relative to non-BPCI hospitals. More research is needed to understand how hospitals are responding to bundled payment incentives and specific practices that contribute to improvements in cost and quality. Economic modelling of costs associated with outcomes reported for type 2 diabetes mellitus (T2DM) patients in the CANVAS and EMPA-REG cardiovascular outcomes trials.
CONCLUSIONS: Models assumed independent, non-recurrent outcomes and were restricted to medical costs directly associated with the trial-reported events. The reductions in CVD events in T2DM patients reported for both CANVAS and EMPA-REG project to a positive cost avoidance for these events in an MCO population. The analysis did not include an assessment of the impact on total cost, as the costs associated with adverse events, drug utilization or other clinical outcomes were not examined. Associations of CAHPS Composites With Global Ratings of the Doctor Vary by Medicare Beneficiaries’ Health Status
This study examines whether the association of care coordination with global ratings of one’s personal doctor varies by number of chronic conditions and self-rated health. Study Design: We used nationally representative Medicare Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey data to evaluate care coordination, doctor communication, getting needed care, getting care quickly, count of 6 chronic conditions (angina, cancer, chronic obstructive pulmonary disease, diabetes, heart attack, stroke), self-rated general health (5-point scale, poor to excellent, scored linearly), and interactions among the...

The development of diabetes complications in GP-centered healthcare.
CONCLUSIONS: The results of the present study indicate that GP-centered healthcare is associated with a delay in the occurrence of serious diabetes complications and reduces the risk of diabetes complications. This may be because GP-centered care is associated with improved coordination of care. PMID: 30020751 [PubMed - in process]
