Factors associated with perforator stroke after selective basilar artery angioplasty or stenting
Conclusions Diabetes, time from last symptom to procedure, and pre-procedure stenosis percentage may be factors affecting the risk of perforator stroke associated with basilar artery angioplasty and/or stenting. These factors should be considered in planning of potential basilar artery angioplasty and/or stenting and prospectively evaluated in future multicenter trials.

Predictors of futile recanalization after endovascular treatment in acute ischemic stroke: a meta-analysis
Conclusion This study identified female sex, comorbidities, admission systolic blood pressure, serum glucose, occlusion site, non-bridging therapy, and post-procedural complication as predictors of futile recanalization, and also confirmed previously reported factors. Further large-scale prospective studies are needed.

The golden hour of stroke intervention: effect of thrombectomy procedural time in acute ischemic stroke on outcome
Conclusions Our findings suggest that extending mechanical thrombectomy procedure times beyond 60 min increases complications and device cost rates while worsening outcomes. These findings can serve as a time frame of when it is prudent to abort a failed thrombectomy case.

P-008 Evaluating Racial/Ethnic Disparity in Endovascular Thrombectomy Outcome for Acute Stroke Patients: 4,763 patients using Premier data 2011 to 2015
ConclusionsThere were no differences in hospital outcome among races/ethnicities in endovascular thrombectomy outcomes for acute stroke patients in the Premier data. Racial/ethnic disparities play role for patients’ selection not for patients’ outcome in endovascular thrombectomy.DisclosuresS. Park: None. M. Pilot: None. M. E-177 Treatment of ischemic stroke in patients diagnosed with covid-19: a matched analysis of the national inpatient sample
ConclusionsIn this matched NIS analysis of patients treated for ischemic stroke with a concomitant COVID diagnosis, we demonstrate that EVT trends towards, although nonsignificant, higher rates of favorable functional outcomes when compared to medically managed patients.Abstract E-177 Table 1Patients with COVID and Acute Ischemic Stroke Comparing medical management vs EVT 1:1 Nearest Neighbor Matching for EVT based on Age, NIHSS, Gender, and Comorbidities Medically Managed N = 495 1 Endovascular ThrombectomyN = 495 1 p-value 2 Age in years at admission 66.82 ± 16.98 (19.00-90.00), (1.77) 65.67 ± 14.81 (28...

O-016 Outcomes after endovascular mechanical thrombectomy for low national institutes of health stroke scale (NIHSS): a multicenter study
ConclusionsMT is an effective and safe treatment for appropriately selected ischemic stroke patients presenting with low NIHSS. Diabetes and prior stroke are predictors of functional dependence at 90 days, and prior stroke is also associated with mortality, suggesting that MT should be offered judiciously to patients with these risk factors. Our findings signal the need for a randomized trial comparing MT versus medical management for LVO patients with low NIHSS.Disclosures I. Abecassis: 2; C; IschemaView (Rapid), Remedy Robotics. E. O-032 Predictors of Poor Outcomes Despite Successful Recanalization in Patients with Acute Ischemic Stroke

Prediction of death after endovascular thrombectomy in the extended window: a secondary analysis of DEFUSE 3 "
Conclusions Despite selective inclusion criteria and overwhelming benefit for EVT, a substantial number of EVT patients in DEFUSE 3 died. The preprocedural variables age, NIHSS, wake-up stroke, and diabetes may predict this risk. Our predictive score provides a basis for future research to determine which factors influence lethal outcome after EVT.

E-249 Analysis of racial and accessibility disparities with mechanical thrombectomy usage for acute ischemic stroke at a single center
ConclusionRacial status was not associated with a difference in clinical presentation, treatment characteristics, nor outcomes. In multivariate analysis, stroke-onset-to-arrival time to our center was the greatest predictor of good neurological outcome. Racial status did not show any significant differences in stroke-onset-to-arrival times at our institution.Disclosures B. Abraham: None. C. Cunningham: None. R. Jaquish: None. W. Troja: None. G. Predictors of early neurologic deterioration (END) following stroke thrombectomy
Conclusions Older patients with acute ischemic stroke who have a history of diabetes or hypertension, with elevated pretreatment SBP and failed reperfusion are at a higher risk of END following stroke thrombectomy for emergent large vessel occlusion.

Predictors and clinical relevance of hemorrhagic transformation after endovascular therapy for anterior circulation large vessel occlusion strokes: a multicenter retrospective analysis of 1122 patients
Conclusions Greater understanding about the predictors and consequences of ICH post endovascular stroke therapy is essential to improve risk assessment, patient selection/clinical outcomes, and early prognostication. Our data suggest that patients with atrial fibrillation are particularly prone to severe ICH and question the ‘benign’ nature of HI suggested by earlier studies.
