Impact of aspiration catheter size on clinical outcomes in aspiration thrombectomy
Conclusions Larger aspiration catheters are associated with a higher rate of FPE, final recanalization with only an aspiration catheter, and in cases with rescue modalities, though with a higher rate of hemorrhagic complications. These findings confirm that aspiration catheter size functions as a variable in aspiration thrombectomy, which should be considered in future study and trial design.

Mildly elevated INR is associated with worse outcomes following mechanical thrombectomy for acute ischemic stroke
Conclusion In patients with stroke treated with MT, mildly elevated INR is associated with worse clinical outcomes after recanalization and may worsen the mortality risk of hemorrhagic transformations.

Endovascular treatment for acute ischemic stroke in patients with tandem lesion in the anterior circulation: analysis from the METRICS study
Conclusion Patients with TL did not differ from those with SO in outcomes after EVT. TL patients with prior IVT had more complete recanalization and mRS 0–2 and IVT was found to be a predictor of good outcome after EVT.

Endovascular treatment of acute tandem lesions in patients with mild anterior circulation stroke
Conclusions EVT for tandem lesions in patients with mild anterior circulation stroke appears to be feasible but may lead to increased rates of sICH. Further studies comparing endovascular with best medical treatment (BMT) especially investigating the risk of periprocedural hemorrhagic complications, are needed.

Safety and efficacy of intensive systolic blood pressure lowering after successful endovascular therapy: a post hoc analysis of the BP TARGET trial
Conclusion The effect of SBP lowering treatment was not modified by age, location of occlusion history of hypertension, intravenous thrombolysis, and admission SBP. Diabetes modified the effect of intensive SBP lowering treatment, and there was a trend towards benefit from intensive SBP treatment in diabetic patients. This finding is hypothesis generating and requires further validation.

Delayed Functional Independence After Neurothrombectomy (DEFIANT) score: analysis of the Trevo Retriever Registry
Conclusion Approximately 45% of patients experience EFI. About one-third of non-early improvers experience DFI. Younger age, lower discharge NIHSS score, and absence of any hemorrhage were independent predictors of DFI among non-early improvers.

Endovascular therapy for acute ischemic stroke in patients with active malignancy: a meta-analysis with trial sequential analysis
Conclusion Although a quarter of patients with active malignancy and AIS regained functional independence, physicians and patients should consider whether to apply EVT in patients with active malignancy.

O01/8 Flow diverting stents in cerebral small calibre vessels (< 2 mm) for aneurysm treatment
ConclusionThe use of FDS in vessels ≤ 2 mm is technically feasible with good aneurysm occlusion rates and an acceptable safety profile. Nevertheless, it is essential to be aware of the of the main complications associated when operating in small diameter vessels.Disclosure of InterestNothing to disclose.

O33/325 The efficacy and safety of Double Stentretrevier as a rescue method after failed thrombectomy
ConclusionRescue Mechanical thrombectomy using double-SR is associated with a higher rate of successful recanalization, first-pass effect, and relatively low rate of hemorrhagic complications. Further randomized control trials are needed to confirm results and long-term outcomes.Disclosure of InterestNothing to disclose.

P005/44 Using the pEGASUS-stent with antithrombogenic properties and single antiplatelet therapy in a case of thrombectomy
IntroductionRescue stenting is used more and more in stroke cases with large-vessel-occlusion or medium-vessel-occlusion refractory to mechanical thrombectomy. The pEGASUS-stent(Phenox) represents a device which is equipped with a hydrophilic polymer coating(HPC) with antithrombogenic properties. The coating allows for implantation under single antiplatelet therapy, thus possibly reducing the risk of bleeding in acute stroke cases.Case HistoryA 79-year-old woman was rushed to emergency room due to left-sided hemiparesis and National-Institutes-of-Health-Stroke-Scale(NIHSS) of 14. Computed-tomography-image showed an occlusi...

P008/85 Transarterial embolization (including hypoglossal artery) of dural arteriovenous fistula with multiple venous varices: Case report
ConclusionDAVF with venous varix is defined as high risk for venous hypertension or hemorrhage. There is a possibility of lower CN injury during embolization for hypoglossal artery, so accurate knowledge of anatomical structures is essential. Transarterial embolization for DAVF with large venous varix is a safe and considerable treatment option, even when fed by hypoglossal artery.Disclosure of InterestNothing to disclose
