E-273 Endovascular treatment for primary isolated distal medium vessel occlusion ischemic stroke: systematic review
ConclusionOur systematic review demonstrates decent rates of functional independence and excellent functional outcome after EVT for isolated primary dMEVO ischemic stroke in the early time window. Mortality and sICH rates were similar to EVT for proximal locations. The study is limited by high heterogenicity and unavailability of comparable medically managed patients with dMEVOs. The EVT should be directly compared with best medical treatment for dMEVOs in a large prospective randomized trial.Disclosures J. Sequeiros: None. A. Rodriguez-Calienes: None. C. Moran-Marinos: None. E-282 Direct to angiosuite strategy versus standard workflow triage for endovascular therapy: systematic review and meta-analysis
ConclusionPatients who underwent the DTAS strategy had a significant reduction in door-to-puncture and door-to- reperfusion times. These time reductions resulted in an increased rate of early neurological and 90-day functional recovery without compromising safety in LVO patients undergoing MT.Disclosures M. E-194 A randomized double-blind placebo-controlled trial of prophylactic antidepressant medication for the prevention of depression post aneurysmal subarachnoid hemorrhage
ConclusionsThe initial results of this ongoing study illustrate the feasibility and challenges of mental health screening and prophylactic randomized treatment in the acute care setting for post-SAH patients. Recruitment has been a challenge in this patient population. Here we describe the trends we see in outcomes at 12-months compared to baseline. These trends suggest that despite clinical improvement there may be an increase in depression and anxiety development one-year post-aSAH.Disclosures J. Keen: None. D. Lim: None. K. Prijoles: None. H. E-215 Single stent assisted coiling for wide neck bifurcation aneurysms: a multi-center core lab validated analysis demonstrating safety and efficacy
ConclusionThis study represents the first multi-center, core-lab validated data of the safety and efficacy of the L-stenting single stent technique for wide-neck bifurcation aneurysms. Further prospective studies in larger cohorts are necessary to compare the efficacy and safety of this treatment to other endovascular techniques for treatment of wide neck bifurcation aneurysms.Disclosures N. E-222 How far is too far in stroke thrombectomy? from xl to xs vessel occlusions with radially adjustable stent-retrievers - tigertriever xl and tigertriever 13
ConclusionMechanical thrombectomy for both primary or secondary DMVO seems feasible and as safe as for LVO. Our initial experience using the Tigertriever13 is of special interest as it shows we can potentially significantly expand AIS population that can benefit from mechanical thrombectomy treatment.Disclosures A. E-224 Bilateral external carotid artery sacrifice in massive refractory hemorrhage of the pharynx: a case report
This report suggests that bilateral ECA embolization may be safe in acute settings. In this report, the bleeding did not respond to conservative or surgical management and ECA embolization was used as a life-saving procedure in order to stop massive bleeding. Bilateral external carotid artery embolization is rarely employed because of the high risk of facial necrosis. Additionally, ECAs are also assumed to serve as potential blood reservoirs, particularly in the case of ICA occlusion where the ipsilateral ECA can significantly contribute to intracranial blood flow via collaterals. However, in this report, no complications ...

E-230 NIHSS surrogates as a predictor of 90-day functional outcome after mechanical thrombectomy for M2 middle cerebral artery occlusions
ConclusionCompared to proximal anterior circulation LVO stroke, M2 MCA occlusion strokes presented lower baseline NIHSS while yielding similar NIHSS improvement ranges with MT. NIHSS 24h after MT for M2 MCA occlusion stroke appears to be a similarly strong predictor of a functional outcome as it has been described for proximal anterior circulation LVO strokes.Disclosures M. E-236 Comparison of pipeline vs surpass flow diverter: a single center experience
ConclusionsIn our single-center retrospective experience in the last 5 years, the Pipeline and Surpass were comparable except that the Surpass flow diverter cases tended to have a higher baseline aneurysm width compared to Pipeline. However, the procedural outcomes and complication rates were comparable between both groups. Long-term and follow-up data to be evaluated for efficacy outcomes assessment.Disclosures A. Pandhi: None. E-250 Flow diversion of superior cerebellar artery aneurysms
ConclusionWhile our data is preliminary and reflects the uncommon nature of these aneurysms, flow diversion of SCA aneurysms with PED appears to be carry a high risk of clinical complications and a low likelihood of complete aneurysm occlusion. Caution when pursuing flow diversion in these aneurysms is warranted. Further study in larger cohorts is necessary to better define clinical scenarios in which flow diversion or other interventions for SCA aneurysms should be considered.Disclosures D. Lauzier: None. S. E-255 Catheter blood vessel ratio for middle cerebral artery occlusions requiring mechanical thrombectomy: a propensity adjusted analysis
ConclusionLower CVR is associated with an increase odd of ICH following mechanical thrombectomy for M1 occlusions. However, this association appears to be confounded by number of passes. Additionally, CVR does improve the predictive value of linear and logistic models to predict NIHSS at discharge and TICI score. Larger prospective trials are warranted to estimate the impact of CVR for mechanical thrombectomy.Disclosures J. Catapano: None. A. Naik: None. M. Pacault: None. R. Sigh: None. H. Stonnington: None. Outcomes of endovascular thrombectomy in patients selected by computed tomography perfusion imaging - a matched cohort study comparing nonagenarians to younger patients
Conclusions Nonagenarians were noted to have greater mortality and sICH rates following EVT compared with matched younger patients, which may be ameliorated by selecting patients with smaller CTP core volumes. Nonagenarians undergoing EVT had similar rates of successful reperfusion and functional independence compared with the younger cohort.
