Chagas disease
Publication date: Available online 30 June 2017 Source:The Lancet Author(s): José A Pérez-Molina, Israel Molina Chagas disease is an anthropozoonosis from the American continent that has spread from its original boundaries through migration. It is caused by the protozoan Trypanosoma cruzi, which was identified in the first decade of the 20th century. Once acute infection resolves, patients can develop chronic disease, which in up to 30–40% of cases is characterised by cardiomyopathy, arrhythmias, megaviscera, and, more rarely, polyneuropathy and stroke. Even after more than a century, many challenges remain unresolved...

Investing in non-communicable diseases: an estimation of the return on investment for prevention and treatment services
Publication date: Available online 5 April 2018 Source:The Lancet Author(s): Melanie Y Bertram, Kim Sweeny, Jeremy A Lauer, Daniel Chisholm, Peter Sheehan, Bruce Rasmussen, Senendra Raj Upreti, Lonim Prasai Dixit, Kenneth George, Samuel Deane The global burden of non-communicable diseases (NCDs) is growing, and there is an urgent need to estimate the costs and benefits of an investment strategy to prevent and control NCDs. Mortality and morbidity in acutely ill adults treated with liberal versus conservative oxygen therapy (IOTA): a systematic review and meta-analysis
This study is registered with PROSPERO, number CRD42017065697. Findings 25 randomised controlled trials enrolled 16 037 patients with sepsis, critical illness, stroke, trauma, myocardial infarction, or cardiac arrest, and patients who had emergency surgery. Compared with a conservative oxygen strategy, a liberal oxygen strategy (median baseline saturation of peripheral oxygen [SpO2] across trials, 96% [range 94–99%, IQR 96–98]) increased mortality in-hospital (relative risk [RR] 1·21, 95% CI 1·03–1·43, I 2=0%, high quality), at 30 days (RR 1·14, 95% CI 1·01–1·29, I 2=0%, high quality), and at longest follow...

Effects of aspirin on risks of vascular events and cancer according to bodyweight and dose: analysis of individual patient data from randomised trials
Publication date: Available online 17 July 2018Source: The LancetAuthor(s): Peter M Rothwell, Nancy R Cook, J Michael Gaziano, Jacqueline F Price, Jill F F Belch, Maria Carla Roncaglioni, Takeshi Morimoto, Ziyah MehtaSummaryBackgroundA one-dose-fits-all approach to use of aspirin has yielded only modest benefits in long-term prevention of cardiovascular events, possibly due to underdosing in patients of large body size and excess dosing in patients of small body size, which might also affect other outcomes.MethodsUsing individual patient data, we analysed the modifying effects of bodyweight (10 kg bands) and height (10 cm ...
