Prevalence, risk determinants and consequences of atrial fibrillation in rheumatic heart disease: 6 years hospital based-Himachal Pradesh- Rheumatic Fever/Rheumatic Heart Disease (HP-RF/RHD) Registry
Conclusion AF is common in RHD patients and is significantly associated with heart failure and systemic thromboembolism. Age, mitral stenosis severity, tricuspid regurgitation and LA size were independently associated with AF.

Gender Disparities with the Use of Percutaneous Left Ventricular Assist Device in Patients Undergoing Percutaneous Coronary Intervention Complicated by Cardiogenic Shock: From pLVAD Working Group
Conclusions Despite the small number of cohorts, this study did not reveal any significant differences among gender with the use of percutaneous left ventricular assist devices for PCI in patients with acute myocardial infarction complicated by CS. However, initiation of Impella prior to PCI may be associated with improved mortality and morbidity in both genders.

Sacubitril/Valsartan: A Novel Angiotensin Receptor-Neprilysin Inhibitor
Conclusion Effective reduction of blood pressure to accepted goals is the key to reduce the risk of CV events and stroke. Dual inhibition of neprilysin and the angiotensin receptor with sacubitril/valsartan may represent an attractive and serendipitous therapeutic approach for a range of CV diseases, including hypertension and HF, in which vasoconstriction, volume overload and neuro-hormonal activation play a part in pathophysiology. Sacubitril/Valsartan appears to be more efficacious in reducing blood pressure than currently available ACEi and ARBs with a similar safety and tolerability profile. Besides, pleiotropic benef...

Salt Sensitivity and its implication in clinical practice
Publication date: Available online 10 October 2017 Source:Indian Heart Journal Author(s): Sundeep Mishra, Shahu Ingole, Rishi Jain Hypertension (HTN) is a complex multi-factorial disease and is considered as one of the foremost modifiable risk factors for stroke, heart failure, ischemic heart disease and renal dysfunction. Over the past century, salt and its linkage to HTN and cardiovascular (CV) mortality has been the subject of intense scientific research. There is now consensus that different individuals have different susceptibilities to blood pressure (BP)-raising effects of salt and is called as salt sensitivity. Va...

Pulse wave analysed cardiovascular parameters in young first degree relatives of type 2 diabetics- a cross-sectional study
Conclusions Young, sedentary, non-obese FDRs of T2D have adverse cardiovascular profile which is suggested to worsen before or with onset of T2DM and definitely need attention for life style modification as primary prevention.

Clinical – microbiological characterization and risk factors of mortality in infective endocarditis from a tertiary care academic hospital in Southern India
CONCLUSIONS This study reiterates the persistent dominance of rheumatic heart disease in the population studied and α − haemolytic Streptococci as the commonest responsible microorganism.

Simplified Percutaneous Closure of Patent Foramen Ovale and Atrial Septal Defect With Use of Plain Fluoroscopy: Single Operator Experience in 110 Consecutive Patients
Conclusion Percutaneous placement of an Amplatzer occluder was safe and effective with use of local anesthesia and fluoroscopy alone. There were no recurrent strokes over >4years. Migraine relief was reported by >80% of patients.

High versus Low dose Statin Therapy in Indian Patients with Acute ST-Segment Elevation Myocardial Infarction Undergoing Thrombolysis
Conclusions High-dose atorvastatin did not show significant difference of MACEs in STEMI patients undergoing thrombolysis but showed significant improvement in immediate coronary flow depicted by ST-segment resolution. This benefit of high dose statin is to be weighed against greater myalgia, drug discontinuation and cost in Indian patients.

Clinical outcome in nonagenarians undergoing transcatheter valve replacement
Conclusion Our case series demonstrate that even with elevated comorbidity index, clinical endpoints and valve-associated results are relatively favorable in nonagenarians treated with TAVR.

Polymorphisms of cytochrome P450 are potential candidates that could potentially help clinicians on the treatment of cardiovascular diseases among Asian populations
Publication date: Available online 24 February 2017 Source:Indian Heart Journal Author(s): López-Hernández Daniel There are significant ethnic differences in the metabolism of some drugs that can be explained by cytochrome P450 polymorphisms and, in particular, the CYP2D6 isozyme. In addition, among patients who are not carriers of the CYP2C19 loss-of-function alleles, there is a reduced risk of stroke when clopidogrel is taken with aspirin compared to aspirin alone. These findings demonstrate that there is inter-ethnic variability in the metabolism of some drugs, which results in different responses to cardiovascular d...

Primary percutaneous coronary intervention for acute ST elevation myocardial infarction: Outcomes and determinants of outcomes: A tertiary care center study from North India
Conclusion Primary PCI for acute STEMI is feasible in our setup and associated with high success rate, low mortality in non-shock patients and low complication rates.

Association of syntax score with short-term outcomes among acute ST-elevation myocardial infarction patients undergoing primary PCI
Conclusions The syntax score is associated with 30-day mortality in patients with STEMI undergoing primary PCI. In those discharged, it is associated with risk of MACE at 30 days.

Clinical presentation, management, and outcomes in the Indian Heart Rhythm Society-Atrial Fibrillation (IHRS-AF) registry
Conclusions In India, AF patients are younger and RHD is still the most frequent etiology. Almost two-third of the patients have persistent/permanent AF. At one-year follow-up, there is a significant mortality and morbidity in AF patients in India.

A rare case of right ventricular myxoma causing recurrent stroke
We present a 62-year-old lady admitted in our hospital with two episodes of acute ischemic stroke about 2 weeks apart. She was evaluated for acute ischemic stroke and was thrombolysed for recent stroke in right MCA territory first time. On further evaluation, she was found to have a RVOT mass. A transthoracic and transesophageal echocardiogram revealed a PFO and a large, 5.1cm×2.3cm, ovoid, well circumscribed, echogenic mass in the right ventricle outflow tract attached by small pedicle to the ventricular side of anterior tricuspid leaflet, partly obstructing the right ventricular outflow tract and protruding through the ...

Bilateral spontaneous internal carotid artery dissection managed with endovascular stenting – A case report
We describe a patient with ischemic stroke from spontaneous bilateral ICAD with completely occluded left ICA. Having failed medical therapy with antiplatelets and anticoagulants due to extensive loss of carotid vascular supply, he was managed successfully with endovascular stenting with good neurological recovery.
