Abstract 267: A Single-page Best Practice Algorithm Reduces Stroke Risk and Improves Arrhythmia Management Decisions in Patients With Acute Atrial Fibrillation: A Quality Improvement Project Session Title: Poster Session PM
Conclusions: Using established quality improvement methodology and cost-neutral multi-disciplinary expertise, this novel management algorithm has significantly improved the quality and safety of care for patients with acute AF at NMUH. Prospective analysis of long-term adverse outcomes is now required to establish morbidity or mortality benefit.

Contemporary Trends in Oral Anticoagulant Prescription in Atrial Fibrillation Patients at Low to Moderate Risk of Stroke After Guideline-Recommended Change in Use of the CHADS2 to the CHA2DS2-VASc Score for Thromboembolic Risk Assessment: Analysis From the National Cardiovascular Data Registrys Outpatient Practice Innovation and Clinical Excellence Atrial Fibrillation Registry Original Articles
Conclusions— Adoption of the CHA2DS2-VASc score reclassifies 64.5% of patients with AF with low CHADS2 scores into a class I indication for OAC prescription. Overall OAC prescription increased between 2011 and 2014.

While the number of CPMs continues to increase, model performance is often inadequately reported and calibration is infrequently assessed. More work is needed to understand the potential impact of this literature.

Conclusion: Knowledge of indications for a primary prevention ICD is poor and a recognized barrier among physicians who may refer patients for device therapy. Adequate knowledge translation of ICD guidelines is crucial in order to improve ICD utilization.

Conclusions— AF diagnosed after stroke is an important hallmark of recurrent stroke risk. Increasing the low utilization of cardiac monitoring after stroke could identify undiagnosed AF earlier, leading to appropriate oral anticoagulation treatment and a reduction in stroke/TIA recurrence.

Conclusion: Focus group participants expressed a fear of debilitating stroke, with varying levels of acceptance of inconveniences and potential bleeding associated with blood thinners. Among a subset of participants, there was willingness to consider new stroke prevention therapies; however, this was preferred with a known, trusted physician. Subspecialized clinicians offering new therapies may be challenged to establish perceived mutual respect with patients, traditionally built over time, and this may be required for patients to consider new therapies. Abstract 176: Variation in the Content and Timing of Informed Consent in Cardiovascular Procedures: An Opportunity to Improve Decision-making Session Title: Poster Session I
Conclusion: We observed notable variation in the content, legibility and timing of informed consent documents within and across procedures. These components are necessary, though may not be sufficient, to support a high-quality informed consent process. Our results highlight opportunities for improving informed consent. Standardization of content and increased time for patients to consider the risks, benefits, and alternatives of elective procedures may result in higher quality decision-making and facilitate patient autonomy.

Does Sex Affect Anticoagulant Use for Stroke Prevention in Nonvalvular Atrial Fibrillation?: The Prospective Global Anticoagulant Registry in the FIELD-Atrial Fibrillation Original Articles
Conclusions— These contemporary global data show that anticoagulant use for stroke prevention is no different in men and women with nonvalvular AF. Thromboprophylaxis was, however, suboptimal in substantial proportions of men and women, with underuse in those at moderate-to-high risk of stroke and overuse in those at low risk. Conclusions— AF is common clinical problem with significant morbidity and mortality. Get With The Guidelines-AFIB is a national hospital-based AF quality improvement program designed to increase adherence to evidence-based guidelines for AF.
