Huge Free-Floating Thrombus in the Internal Carotid Artery
A 70-year-old male patient presented with very short recurrent speech difficulty and left-sided monocular transient visual loss. The patient was diagnosed with Crohn's disease and had hemicolectomy due to obstruction 10 years ago. He also had a history of coronary artery disease, hypertension, left femoral vein thrombosis, and 50-pack-year smoking. He had no stroke attack before. He was taking regularly dual antiplatelet therapy and azathioprine.

Uncommon Causes of Cerebral Microbleeds
Cerebral microbleeds (CMBs) are small and round perivascular hemosiderin depositions detectable by gradient echo sequences or susceptibility-weighted imaging. Cerebral microbleeds are common among patients with hypertension, cerebral ischemia, or cerebral amyloid angiopathy. In this article, we describe uncommon causes of CMBs.

Multiple Lacunar Infarcts in Paroxysmal Nocturnal Hemoglobinuria

Cardiovascular Responses Associated with the Moving Beans Task: Influence of Psychological Characteristics
High blood pressure (BP) after stroke is associated with a poor outcome. However, exercise training or speech therapy for patients with stroke can raise the BP. The aim of this study was to examine cardiovascular responses during the moving beans task (MBT) used in occupational therapy and to study the influence of psychological characteristics on cardiovascular responses during this task in healthy subjects.

Large Amount of Cannabis Ingestion Resulting in Spontaneous Intracerebral Hemorrhage: A Case Report
Although multiple cases of cannabis-associated ischemic stroke have been reported, there are only 2 reported cases of hemorrhagic stroke with an associated cerebral vasoconstriction. To our knowledge, we present the first case of basal ganglia hemorrhage after a large-volume oral ingestion of cannabis without other identified risk factors. In our case, cerebral digital subtraction angiography within 24 hours of presentation did not reveal vasoconstriction leading to a possible alternative explanation for hemorrhagic stroke, including cannabis-induced transient arterial hypertension and autoregulation disruption.

Endovascular thrombectomy in a patient with acute left internal carotid artery occlusion receiving rivaroxaban
The 78-year-old female patient came to our emergency department (ED) due to acute consciousness disturbance, global aphasia and right-sided weakness onset within 3 hours. She had hypertension, persistent non-valvular atrial fibrillation treated with rivaroxaban 10 mg/day, and previous stroke with sequela of left-sided paralysis. Her pre-stroke modified Rankin Scale (mRS) was 2. NIHSS score was 32 at ED, partially contributed by previous stroke. And her creatinine clearance was 50 ml/min, international normalized ratio (INR) was 2.38.

Resistant hypertension as a poor prognosticator for acute ischemic stroke
【Purpose】Some patients have a resistant hypertension even though they take three or more classes of antihypertensive drugs. There was not any study related to the outcomes of ischemic stroke in patients with resistant hypertension.

Calcified Amorphous Tumor Causing Shower Embolism to the Brain: A Case Report with Serial Echocardiographic and Neuroradiologic Images and a Review of the Literature
An 89-year-old woman with chronic atrial fibrillation, hypertension, chronic heart failure, and dementia was admitted to our hospital due to multiple small cerebral and cerebellar infarctions. Transthoracic echocardiogram revealed a floating calcified mass lesion arising from the endocardium of the posterior portion of the mitral annulus with mitral annular calcification. Furthermore, the mass had a heterogeneity of the echogenicity. The mass was diagnosed as a calcified amorphous tumor based on specific echocardiographic features.

A Case of Hypertensive Encephalopathy with Enlarged Optic Nerve Sheath Measured by Transorbital Sonography
This case report describes our experience in using transorbital sonography to evaluate pathological changes in the central nervous system in hypertensive encephalopathy. A 49-year-old man with nausea, headache, and mild confusion was diagnosed with hypertensive encephalopathy by brain magnetic resonance imaging (MRI), which revealed vasogenic edema in the bilateral thalamus and the brain stem. Lumbar puncture showed no severe intracranial hypertension. Transorbital sonography showed an increase in the optic nerve sheath diameter (ONSD).

Effective ADAPT Thrombectomy in a Patient with Acute Stroke due to Cardiac Papillary Elastofibroma: Histological Thrombus Confirmation
A 75-year-old man with hypertension and atrial fibrillation was admitted to our emergency room with right-sided hemiplegia and complete aphasia (National Institutes of Health Stroke Scale [NIHSS] score = 18). A noncontrast computed tomography scan showed a slight hypodensity in the left insular region and a bright hyperdense sign in the M1 tract of the left middle cerebral artery (MCA). Angio-CT confirmed an occlusion of the M1 tract of the MCA. Magnetic resonance diffusion-weighted imaging/p erfusion-weighted imaging was obtained and revealed a mismatch in the left parietal cortical region.

ACE Gene in Egyptian Ischemic Stroke Patients
Angiotensin-1-converting enzyme (ACE) is a crucial player in vascular homeostasis and in the pathogenesis of atherosclerosis and hypertension. The present study was conducted to determine whether there is an association between the ACE insertion/deletion (I/D) polymorphism and ischemic stroke in Egyptian population. Also, we analyzed the ACE gene I/D polymorphism as a risk factor for small-vessel (SV) versus large-vessel (LV) disease.
