Modification of Outcomes With Aspirin or Apixaban in Relation to Female and Male Sex in Patients With Atrial Fibrillation: A Secondary Analysis of the AVERROES Study Brief Reports
Conclusions— Female patients with atrial fibrillation had higher ischemic stroke rates compared with male patients, but the relative effects of apixaban compared with aspirin on both ischemic stroke and bleeding were similar in men and women.

Warfarin Versus Aspirin for Prevention of Cognitive Decline in Atrial Fibrillation: Randomized Controlled Trial (Birmingham Atrial Fibrillation Treatment of the Aged Study) Clinical Sciences
Conclusions— We found no evidence that anticoagulation confers clinically important protection over aspirin against cognitive decline as measured by the Mini-Mental State Examination in atrial fibrillation in the first 33 months of treatment other than that provided by preventing clinical stroke. Clinical Trial Registration— URL: http://www.controlled-trials.com. Unique identifier: ISRCTN89345269.

C-Reactive Protein as a Prognostic Marker After Lacunar Stroke: Levels of Inflammatory Markers in the Treatment of Stroke Study Clinical Sciences
Conclusions— Among recent lacunar stroke patients, hsCRP levels predict the risk of recurrent strokes and other vascular events. hsCRP did not predict the response to dual antiplatelets. Clinical Trial Registration— URL: http://www.clinicaltrials.gov. Unique identifier: NCT00059306.

Antiplatelet Treatment for Prevention of Cerebrovascular Events in Patients With Vascular Diseases: A Systematic Review and Meta-Analysis Clinical Sciences
Conclusions— DAPT with clopidogrel and aspirin compared with aspirin effectively reduces the risk of total and ischemic stroke in the overall cohort consisting of patients with cardiovascular disease without increase in intracranial hemorrhage, as well as decreases the risk of a recurrent total stroke in patients with a previous stroke/transient ischemic attack. Our meta-analysis suggests that DAPT including low-dose aspirin (75–100 mg) and clopidogrel (75 mg) should be further investigated as a strategy to reduce recurrent strokes. Antiplatelet Loading Improves Behavioral Outcome in a Rabbit Model of Stroke Brief Reports
Conclusions— This study suggests that usual dual antiplatelet loading is clinically beneficial in a validated model of acute stroke. Study of usual dual antiplatelet loading in acute stroke is warranted to provide treatment to stroke victims ineligible for current therapies.

Evaluation of Rural Stroke Services: Does Implementation of Coordinators and Pathways Improve Care in Rural Hospitals? Basic Sciences
Conclusions— Investment in clinical coordinators who implemented organizational change, together with increased clinician resources, effectively improved stroke care in rural hospitals, resulting in more patients being discharged home.

Urgent Best Medical Therapy May Obviate the Need for Urgent Surgery in Patients With Symptomatic Carotid Stenosis Clinical Sciences
Conclusions— CEA can be performed in the subacute period without significantly increasing the operative risk. The urgent best medical treatment was associated with significant reduction in the risk of early NR in CEA patients. It seems that urgent aggressive best medical treatment may obviate the need for urgent CEA.

Amino Terminal Pro-B-Type Natriuretic Peptide, Secondary Stroke Prevention, and Choice of Antithrombotic Therapy Clinical Sciences
Conclusions— For secondary stroke prevention, elevated NT-proBNP concentrations may identify a subgroup of ischemic stroke patients without known atrial fibrillation, about 5% based on the current study, who may benefit more from anticoagulants than antiplatelet agents. Clinical Trial Registration— This trial was not registered because enrollment began before 2005.

Effect of Low-Dose Aspirin on Functional Outcome From Cerebral Vascular Events in Women Original Contributions
Conclusions— Results from this large randomized clinical trial provide evidence that 100 mg of aspirin every other day may reduce the risk of ischemic cerebral vascular events but does not have differential effects on functional outcomes from stroke.
