Fractional Flow Reserve-Guided PCI or Coronary Bypass Surgery for 3-Vessel Coronary Artery Disease: 3-Year Follow-Up of the FAME 3 Trial
CONCLUSIONS: At 3-year follow-up, there was no difference in the incidence of the composite of death, MI, or stroke after FFR-guided PCI with current-generation drug-eluting stents compared with CABG. There was a higher incidence of MI after PCI compared with CABG, with no difference in death or stroke. These results provide contemporary data to allow improved shared decision-making between physicians and patients with 3-vessel coronary artery disease.REGISTRATION: URL: https://www.CLINICALTRIALS: gov; Unique identifier: NCT02100722.PMID:37602376 | DOI:10.1161/CIRCULATIONAHA.123.065770

Everolimus-Eluting Stents or Bypass Surgery for Multivessel Coronary Artery Disease: Extended Follow-up Outcomes of Multicenter Randomized Controlled BEST Trial
CONCLUSIONS: In patients with multivessel coronary artery disease, there were no significant differences between PCI and CABG in the incidence of major adverse cardiac events, safety composite end point, and all-cause mortality during extended-follow-up.PMID:36121700 | DOI:10.1161/CIRCULATIONAHA.122.062188

Icosapent Ethyl Reduces Ischemic Events in Patients with a History of Prior Coronary Artery Bypass Grafting: REDUCE-IT CABG
Conclusions: In REDUCE-IT patients with a history of CABG, treatment with icosapent ethyl was associated with significant reductions in first and recurrent ischemic events.PMID:34710343 | DOI:10.1161/CIRCULATIONAHA.121.056290

Postoperative Atrial Fibrillation and Long-Term Risk of Stroke After Isolated Coronary Artery Bypass Graft Surgery.
CONCLUSIONS: Patients with pAF after coronary artery bypass grafting are at higher risk of CVA. These findings challenge the notion that pAF is a benign complication. PMID: 33017213 [PubMed - as supplied by publisher]

Impact of Left Atrial Appendage Exclusion on Short Term Outcomes In Isolated Coronary Artery Bypass Graft Surgery.
Conclusions: LAA exclusion during isolated CABG in patients with AF is associated with a higher rate of 30-day readmission. Post-operative measures to mitigate the loss of the hormonal and hemodynamic effects of the LAA may increase the therapeutic benefit of this procedure. Comparison of a Complete Percutaneous versus Surgical Approach to Aortic Valve Replacement and Revascularization in Patients at Intermediate Surgical Risk: Results from the Randomized SURTAVI Trial.
CONCLUSIONS: For patients at intermediate surgical risk with severe AS and non-complex CAD (SYNTAX score ≤ 22), a complete percutaneous approach of TAVR and PCI is a reasonable alternative to SAVR and CABG. Five-Year Outcome after Off-Pump or On-Pump Coronary Artery Bypass Grafting in Elderly Patients.
CONCLUSIONS: In elderly patients ≥75 years, the five-year survival rates as well as the combined outcome of death, myocardial infarction and repeat revascularization were similar after on-pump and off-pump CABG. Incomplete revascularization was associated with a lower five-year survival rate, irrespective of the type of surgery. Multiple Arterial Grafting Is Associated With Better Outcomes for Coronary Artery Bypass Grafting Patients.
CONCLUSIONS: CABG with 3 arterial grafts was not associated with increased in-hospital death nor with better clinical outcomes at 8-year follow-up, compared with CABG with 2 arterial grafts. MAG was associated with superior outcomes compared with SAG. PMID: 30474420 [PubMed - in process]

B-Type Natriuretic Peptide Assessment in Patients Undergoing Revascularization for Left Main Coronary Artery Disease: Analysis from the EXCEL Trial.
Conclusions -In the EXCEL trial, elevated baseline BNP levels in patients with LMCAD undergoing revascularization were independently associated with long-term mortality but not non-fatal adverse ischemic or bleeding events. The relative long-term outcomes after PCI vs. CABG for revascularization of LMCAD may be conditioned by the baseline BNP level. Clinical Trial Registration -URL: http://www.clinicaltrials.gov. Second Arterial versus Venous Conduits for Multi-Vessel Coronary Artery Bypass Surgery in California.
Conclusions -Second arterial conduit use in California is low and declining, but arterial grafts were associated with significantly lower mortality and fewer cardiovascular events. A right internal thoracic artery graft offered no benefit over that of a radial artery, but did increase risk of sternal wound infection. These findings suggest surgeons should consider lowering their threshold for using arterial grafts, and the radial artery may be the preferred second conduit.
