Stroke After Left Ventricular Assist Device Implantation: Outcomes in the Continuous-Flow Era.
CONCLUSIONS: Stroke while on CF-LVAD support was associated with significant mortality. To reduce the risk of stroke, it is essential to further elucidate risk factors, to optimize anticoagulation, and to further understand the impact of LVAD-related infections. Outcomes of Heart Transplantation in Small Children Bridged With Ventricular Assist Devices.
CONCLUSIONS: Small children bridged to HTx with a VAD have early survival rates that are comparable to nonbridged children; however, this is achieved at the expense of a higher rate of stroke. Identifying the risk factors for early death and stroke in small children bridged to HTx with VADs is prudent as more experience with this patient population accumulates. The Subclavian Intraaortic Balloon Pump: A Compelling Bridge Device for Advanced Heart Failure.
CONCLUSIONS: The SC-IABP provided excellent hemodynamic support with minimal morbidity and mortality, allowed for extensive rehabilitation, and permitted more than 90% of patients to receive their intended therapy. Therefore, SC-IABP is a compelling bridge device for patients with advanced congestive heart failure. PMID: 26228596 [PubMed - as supplied by publisher]

Sixth Annual Pediatric Interagency Registry for Mechanical Circulatory Support (Pedimacs) Report
CONCLUSIONS: This Sixth Pedimacs Report demonstrates the continued evolution of the pediatric field. The complexity of cardiac physiologies and anatomical constraint mandates the need for multiple types of devices utilized (PC, PP, IC). Detailed analyses of each device type in this report provide valuable information to further advance the care of this challenging and vulnerable population.PMID:36402175 | DOI:10.1016/j.athoracsur.2022.10.042

Outcomes of Atrioventricular Valve Operation in Patients With Fontan Circulation.
CONCLUSIONS: The AV valve operation done before, during, or after the Fontan operation is associated with low operative mortality but a high reoperation rate with significant risk of late death, transplantation, and persistent AV valve regurgitation. The Effect of Institutional Volume on Complications and Their Impact on Mortality After Pediatric Heart Transplantation.
CONCLUSIONS: No association was observed between institutional volume and adjusted or unadjusted long-term survival. High-volume institutions have a significantly lower rate of postoperative complications after pediatric OHT. This association does not correlate with increased subsequent mortality in low-volume institutions. Given these findings, strategies integral to the allocation of allografts in adult transplantation, such as regionalization of care, may not be as relevant to pediatric OHT. Outcomes on Continuous Flow Left Ventricular Assist Devices: A Single Institutional 9-Year Experience.
CONCLUSIONS: These data indicate excellent survival for BTT and DT patients on long-term LVAD support. However, for LVAD therapy to become a plausible alternative to heart transplantation, we need to further decrease the incidence of postoperative complications. Surgical outcomes in Behcet's disease patients with severe aortic regurgitation.
CONCLUSIONS: The findings suggest that mechanical root replacement combined with a low post-operative C-reactive protein level maintained through adjunctive immunomodulation therapy may lead to optimal surgical outcomes in Behcet's disease associated with severe aortic regurgitation. Successful heart transplantation in a patient with Loeys-Dietz Syndrome.
Abstract Our patient is a 45-year-old woman with a history of Loeys-Dietz syndrome (LDS), vertebral artery dissection, stroke, prior atrial septal defect (ASD) repair, and spontaneous left main coronary artery dissection re-vascularized with coronary artery bypass grafting who progressed to end-stage heart failure. She was listed for cardiac transplantation and a suitable organ became available. She underwent heart transplantation over one year ago successfully. This is the first reported case of a successful heart transplantation in an individual with Loeys-Dietz syndrome. PMID: 30513314 [PubMed - as supplied by publisher]

The Society of Thoracic Surgeons Intermacs Database Annual Report: Evolving Indications, Outcomes, and Scientific Partnerships.
CONCLUSIONS: With the evolution of MCS, patient phenotype and outcomes are also changing over time. CF LVAD support is increasingly being used in the less ill patient phenotype and more patients are supported for destination therapy. Mean survival is now approaching 5 years, but adverse events, especially neurologic events, continue to have a detrimental impact on the success of CF LVAD support. Long-Term Single Centre Outcomes of Patients with Chronic Renal Dialysis Undergoing Cardiac Surgery.
CONCLUSIONS: Cardiac surgery in DD-CRF patients is associated with high morbidity and mortality. Interestingly, overall mortality was mainly none cardiac-related, where elderly patients or those presented with CHF had the worst life expectancy. However, subsequent kidney transplantation positively impact long-term survival in such patients. PMID: 31563486 [PubMed - as supplied by publisher]

Cardiac Transplantation Using Hearts with Transient Dysfunction: Role of Takotsubo Like Phenotype Running Head: Takotsubo Like Syndrome in Donor Hearts.
CONCLUSIONS: In the largest analysis of transplanted donor hearts with transient LVSD, 1 in 4 had RWMAs on the initial TTE; but this was not associated with adverse post-transplant outcomes. Donor hearts with initial LVSD should be pursued irrespective of TTS-like hypokinesis or other RWMAs. PMID: 31816283 [PubMed - as supplied by publisher]

A Frailty Screening Tool for Patients undergoing Orthotopic Heart Transplant.
CONCLUSIONS: The OHT frailty screening tool is highly predictive of adverse post-transplant outcomes. This screening tool may provide a framework to improve enhance existing risk stratification tools and improve overall resource utilization. PMID: 32622795 [PubMed - as supplied by publisher]

The Specialized Donor Care Facility (SDCF) Model and Advances in Management of Thoracic Organ Donors.
CONCLUSIONS: Lack of high-quality prospective evidence results in conflicting practices across organ procurement organizations and best practices remain controversial. Future studies should focus on prospective, randomized investigations to evaluate donor management strategies. The SDCF model fosters a collaborative environment that encourages academic inquiry and is an ideal setting for these investigations. PMID: 33421385 [PubMed - as supplied by publisher]
