Impact of Perioperative Stroke on Mid Term Outcomes after Transcatheter Aortic Valve Replacement.
CONCLUSIONS: Stroke increases short term and mid-term mortality following TAVR. Risk prediction for neurologic events in TAVR could aid framework for patient selection, further improving outcomes. PMID: 32151578 [PubMed - as supplied by publisher]

Transcatheter and Surgical Aortic Valve Replacement in Patients with Recent Acute Heart Failure.
CONCLUSIONS: Recent AHF is associated with increased risk of mortality and morbidity after SAVR and TAVR. These findings suggest that AS patients should be referred for invasive treatment before the development of clinically evident heart failure. PMID: 31288017 [PubMed - as supplied by publisher]

Comparative Outcomes of Transcarotid and Trans-subclavian Transcatheter Aortic Valve Replacement.
CONCLUSIONS: The transcarotid and the trans-subclavian approaches have similar safety and efficacy outcomes. The transcarotid approach had a shorter procedure duration and a trend towards lower fluoroscopy duration and radiation exposure. PMID: 31279787 [PubMed - as supplied by publisher]

Association Between Red Blood Cell Transfusion and Clinical Outcomes Among Patients Undergoing Transcatheter Aortic Valve Replacement.
CONCLUSIONS: PRBC transfusion in patients undergoing TAVR is associated with an increased incidence of adverse outcomes during hospitalization, at 30-day and 1-year follow-up. PMID: 30552884 [PubMed - as supplied by publisher]

Cognition and Cerebral Infarction in Older Adults After Surgical Aortic Valve Replacement.
CONCLUSIONS: In high-risk, aged participants undergoing surgical AVR for aortic stenosis, post-operative cognitive dysfunction was surprisingly limited and was resolved by 1-year in most. Post-operative cognitive dysfunction at 4-6 weeks was associated with more and larger acute cerebral infarcts. PMID: 30423336 [PubMed - as supplied by publisher]

Preoperative Venoarterial Extracorporeal Membrane Oxygenation Slashes Risk Score in Advanced Structural Heart Disease.
CONCLUSIONS: ECMO can be used as a bridge to heart valve or septal defect surgery in severely decompensated patients. Through recovery of end-organ function, ECMO may allow surgical correction of structural heart disease in patients considered inoperable or convert a salvage situation to an elective operation. Hemiarch Reconstruction Vs Clamped Aortic Anastomosis for Concomitant Ascending Aortic Aneurysm.
CONCLUSIONS: Hemiarch replacement using DHCA with RCP does not increase the risk of operative complications compared to a normothermic, clamped-distal aortic anastomosis, and therefore its use should not be limited when planning complex multi-procedural reconstructions during elective ascending thoracic aortic replacement with concomitant cardiac surgery. Intraoperative Cerebral Perfusion Disturbances During Transcatheter Aortic Valve Replacement.
CONCLUSIONS: Cerebral oximetry reacted promptly to rapid ventricular pacing with significant desaturation and hyperemia a common occurrence. However, no association between this intraoperative insult and objective neurologic injury was detected. PMID: 28821337 [PubMed - as supplied by publisher]

Outcomes for Low-Risk Surgical Aortic Valve Replacement: A Benchmark for Aortic Valve Technology.
CONCLUSIONS: Low-risk patients undergoing surgical AVR in the current era have excellent results. The most common complications were atrial fibrillation and bleeding. These real-world results should provide additional context for upcoming transcatheter clinical trial data. PMID: 28610884 [PubMed - as supplied by publisher]

The Carotid Artery As a Preferred Alternative Access Route For Transcatheter Aortic Valve Replacement.
CONCLUSIONS: In our US community hospital setting, TC-TAVR is a safe alternative to TF-TAVR in appropriate patients and has evolved to be our alternative access route of choice if TF access is not feasible. PMID: 28274520 [PubMed - as supplied by publisher]

Prolonged Cross-Clamping During Aortic Valve Replacement Is an Independent Predictor of Postoperative Morbidity and Mortality: Analysis of the Japan Cardiovascular Surgery Database.
CONCLUSIONS: Prolonged ACCD offers an independent predictor of postoperative morbidity and mortality after isolated AVR for AS despite recent technologic advances and surgical refinements. PMID: 27624296 [PubMed - as supplied by publisher]

Aortic Valve Replacement in the Moderately Elevated Risk Patient: A Population-Based Analysis of Outcomes.
CONCLUSIONS: This population-based contemporary assessment suggests moderate-risk patients undergoing AVR experience favorable outcomes. Although increasing PROM is important in preoperative evaluation of risk, preexisting pulmonary hypertension and indication for operation are among other factors that should be considered as TAVR expands into this group of patients. Bicuspid Aortic Insufficiency With Aortic Root Aneurysm: Root Reimplantation Versus Bentall Root Replacement.
CONCLUSIONS: In patients with bicuspid aortic valve AI with root aneurysm, primary cusp repair with root reimplantation achieves equivalent midterm outcomes compared with Bentall root replacement. PMID: 27261086 [PubMed - as supplied by publisher]

Transcatheter Aortic Valve Replacement Versus Aortic Valve Bypass: A Comparison of Outcomes and Economics.
CONCLUSIONS: TAVR and AVB relieve aortic stenosis and have similar and acceptable procedural mortality rates. AVB-treated patients had 1.75 times the STS risk score when compared with the TAVR cohort. Hospital charges for TAVR were nearly twofold those of AVB. Hospital reimbursement was similar, but AVB had two to four times the profit margin of TAVR. Longer follow-up for the TAVR cohort will determine whether survival is comparable to that after AVB at 3 and 5 years. PMID: 26365675 [PubMed - as supplied by publisher]
