Managing uncertainty: Physicians' decision-making for stroke prevention for patients with atrial fibrillation and intracerebral haemorrhage
Conclusion Physicians described the process of deciding on stroke prevention in patients with AF post-ICH as 'challenging' due to considerable 'clinical equipoise'. Key factors that affected decision-making was patient comorbidities, functional status, and patient willingness to engage with oral anticoagulation therapy. Shared decision-making was believed to be beneficial, but physicians believed that the ultimate responsibility to decide on stroke prevention lay with the clinician.PMID:35253141 | DOI:10.1055/a-1789-4824

Risk factors and predictors for venous thromboembolism in people with ischemic stroke: A systematic review
In conclusion, this systematic review informs on several potential risk factors and predictors for VTE in people with ischemic stroke. To improve risk stratification and guide development of risk prediction models, further confirmation is needed, as there were few high-quality studies on each factor.PMID:35815351 | DOI:10.1111/jth.15813

Hypercoagulability on Thromboelastography Can Predict the Functional Outcome in Patients with Acute Ischemic Stroke
CONCLUSION: A R of <5 min was a negative predictor of functional independence. Frequent HT, END, and different stroke etiologies according to the R may be the cause of poor outcome after ischemic stroke.PMID:37130549 | DOI:10.1055/a-2084-5018

Unanswered questions and research priorities to optimise stroke prevention in atrial fibrillation with the new oral anticoagulants.
Abstract This review article discusses the following, as yet unanswered, questions and research priorities to optimise patient management and stroke prevention in atrial fibrillation with the new direct oral anticoagulants (NOACs): 1. In patients prescribed a NOAC, can the anticoagulant effects or plasma concentrations of the NOACs be measured rapidly and reliably and, if so, can "cut-off points" between which anticoagulation is therapeutic (i.e. the "therapeutic range") be defined? 2. In patients who are taking a NOAC and bleeding (e.g. intracerebral haemorrhage), can the anticoagulant effects of the direct NOACs...

Feasibility and cost effectiveness of stroke prevention through community screening for atrial fibrillation using iPhone ECG in pharmacies. The SEARCH-AF study.
Abstract Atrial fibrillation (AF) causes a third of all strokes, but often goes undetected before stroke. Identification of unknown AF in the community and subsequent anti-thrombotic treatment could reduce stroke burden. We investigated community screening for unknown AF using an iPhone electrocardiogram (iECG) in pharmacies, and determined the cost-effectiveness of this strategy.Pharmacists performedpulse palpation and iECG recordings, with cardiologist iECG over-reading. General practitioner review/12-lead ECG was facilitated for suspected new AF. An automated AF algorithm was retrospectively applied to collecte...

The prediction of postoperative stroke or death in patients with preoperative atrial fibrillation undergoing non-cardiac surgery: a VISION sub-study.
CONCLUSIONS: In AF patients, the 3 thromboembolic risk scores performed similarly to the RCRI in predicting death within 30 days and the CHADS2 score was the best predictor of postoperative stroke/death regardless of type of surgery. Improving stroke risk prediction in the general population: Common clinical rules, a new multimorbid index and machine learning based algorithms
Conclusion Complex relationships of various comorbidities uncovered using a ML approach for diverse(and dynamic) multimorbidity changes have major consequences for stroke risk prediction.PMID:33765685 | DOI:10.1055/a-1467-2993

Thrombolytic strategies for ischemic stroke in the thrombectomy era
J Thromb Haemost. 2021 Apr 9. doi: 10.1111/jth.15336. Online ahead of print.ABSTRACTTwenty-five years ago, intravenous thrombolysis has revolutionized the care of patients with acute ischemic stroke. Since 2015, randomized clinical trials have demonstrated that mechanical thrombectomy improves functional outcome in stroke patients over intravenous thrombolysis alone. More recently, three randomized clinical trials have suggested that mechanical thrombectomy alone is non-inferior to a combined strategy with both intravenous thrombolysis and mechanical thrombectomy. In the present review, we will present the last clinical an...
