Anticoagulation use and the risk of stroke and major bleeding in patients on hemodialysis: From the VIVALDI, a population-based prospective cohort study
CONCLUSIONS: Although the non-randomized nature of the study is prone to bias, anticoagulation with VKA was not associated with decreased thromboembolic risk, but associated with increased risk of major bleeding and may be net-harmful to patients with AF on HD.PMID:34418291 | DOI:10.1111/jth.15508

Stroke and Thromboembolism in Patients with Heart Failure and Sinus Rhythm: A Matter of Risk Stratification?
Thromb Haemost. 2022 Jan 19. doi: 10.1055/a-1745-2083. Online ahead of print.ABSTRACTPatients with heart failure (HF) in sinus rhythm (SR) experience an increased incidence of thromboembolic events including stroke. Among patients with HF, high-quality evidence supports the use of oral anticoagulation when atrial fibrillation (AF) is present, but the benefit of anticoagulation in SR in absence of other known indications for anticoagulation is unclear. In four randomized controlled trials (RCTs), warfarin did not improve a composite of clinical outcomes compared with aspirin or placebo in patients with HF with reduced eject...

Active FXI Can Independently Predict Ischemic Stroke in Anticoagulated Atrial Fibrillation Patients: A Cohort Study
CONCLUSION: FXIa present in circulating blood is associated with increased risk of ischemic stroke and cardiovascular death in anticoagulated AF patients during long-term follow-up. FXIa inhibition could be useful in cardiovascular prevention in AF beyond the current oral anticoagulation.PMID:35158400 | DOI:10.1055/s-0042-1742366

Thrombus composition and thrombolysis resistance in stroke
Res Pract Thromb Haemost. 2023 May 16;7(4):100178. doi: 10.1016/j.rpth.2023.100178. eCollection 2023 May.ABSTRACTA State of the Art lecture titled "Thrombus Composition and Thrombolysis Resistance in Stroke" was presented at the ISTH Congress in 2022. Intravenous thrombolysis (IVT) remains the only pharmacologic option to re-establish cerebral perfusion at the acute phase of ischemic stroke. IVT is based on the administration of recombinant tissue plasminogen activator with the objective of dissolving fibrin, the major fibrillar protein component of thrombi. Almost 30 years on from its introduction, although the clinical b...

New oral anticoagulants for stroke prevention in atrial fibrillation: impact of study design, double counting and unexpected findings on interpretation of study results and conclusions.
Abstract Four recently introduced new oral anticoagulants (dabigatran, rivaroxaban, apixaban and edoxaban) have been shown to be at least as efficacious and safe as warfarin for stroke prevention in patients with atrial fibrillation in their respective trials. The first three have been approved, while edoxaban is awaiting regulatory approval. Several guidelines have endorsed the approved new oral anticoagulants over warfarin because of their favourable risk-benefit ratio, low propensity for food and drug interactions, and lack of requirement for routine coagulation monitoring. In this invited review, we summarise ...

Inflammation Markers and Incident Venous Thromboembolism: the REasons for Geographic And Racial Differences in Stroke (REGARDS) Cohort.
CONCLUSION: Higher CRP and lower serum albumin were associated with increased VTE risk, and statistically mediated part of the association of BMI with VTE. These data suggest inflammation may be a potential mechanism underlying the relationship of obesity and VTE risk. This article is protected by copyright. All rights reserved. ASTRAL-R score predicts non-recanalisation after intravenous thrombolysis in acute ischaemic stroke.
In conclusion, the 5-item ASTRAL-R score moderately predicts non-recanalisation at 24 hours in thrombolysed ischaemic strokes. If its performance can be confirmed by external validation and its clinical usefulness can be proven, the score may influence patient selection for more aggressive revascularisation strategies in routine clinical practice. Hypercoagulability and the risk of myocardial infarction and ischemic stroke in young women.
CONCLUSION: In young women, prothrombotic factors are more associated with risk of IS than MI risk, suggesting a different role of hypercoagulability in the mechanism leading to these two diseases. Antibodies against HDL Components in Ischaemic Stroke and Coronary Artery Disease.
In this study, we investigated whether antibodies against HDL components had additional value to conventional cardiovascular risk factors for the diagnosis of ischaemic stroke (IS) and coronary artery disease (CAD). Cross-sectional study was conducted on 53 patients with IS, 51 with CAD and 55 healthy controls, and in vitro studies to validate findings of the clinical study. We determined serum immunoglobulin G (IgG) antibodies against HDL (aHDL), apolipoproteins (aApoA-I, aApoA-II and aApoC-I) and paraoxonase-1 (aPON1) as well as PON1 activity (PON1a), total antioxidant capacity and biomarkers of endothelial activation (s...
